Abstract

Hypertension is a very common disorder in our society and the consumption of antihypertensive agents has been increasing a lot in the last few years. Different classes of drugs to reduce blood pressure are available on the market. The challenge for the prescribers is to determine which antihypertensive drug to use to take care of their patients.Therefore various guidelines exist to guide their choice. Several studies have called the rote of P-blockers in the treatment of hypertension into question over the last few years, creating many controversies among the scientific community. Inthis scientific pandemonium , the main guidelines (British, U.S., European) had to make a decision and this, in a variable way. Beyond an attempt of clarification of the current situation, this paper will also clarify the role of P-blockers in the treatment of specific patients such as patients with COPD, gestational hypertension or heart failure. L'hypertension artériel le est une pathologie très fréquente dans notre société et la consommation d 'agents antihypertenseurs n'a cessé d'augmenter ces dernières années. Il existe actuellement sur le marché différentes classes de méd icaments visant à réduire la tension artérielle. La difficulté pour les prescripteurs sera dès lors d'évaluer quel antihypertenseur choisir pour la prise en charge de leur patient. Différentes guidelines existent pour orienter leur choix. Ces dernières années plusieurs études ont remis en doute la place des P-bloquants dans le traitement de l 'hypertension, créant ainsi de nombreux désaccords parmi la communauté scientifique. Dans ce tohu­ bohu scientifique, les principales guidelines (britannique, US, européennes) ont dû trancher, et ce de manière relativement variable. Au-delà d'une tentative d'éclaircissement de la situation actuelle, ce mémoire précisera la place des P-bloquants dans le traitement de patients particuliers tels que les patients atteints de BPCO, d'hypertension gravidique ou d'insuffisance cardiaque.

Le dopage est un sujet d'actualité touchant aussi bien les athlètes professionnels que les sportifs amateurs. Pour atteindre les marches du podium, la tentation de recourir aux substances illicites n'est pas prête de s'arrêter. Parmi ces substances, la majorité sont des dérivés de médicaments voire des médicaments administrés tels quels.Chaque sport demande des aptitudes différentes : de la force, de l'endurance, de la précision ,... Et chacune d'entre elle peuvent être améliorées grâce à des substances dopantes bien précises. Des substances comme l'EPO et les hormones de croissance n'ont plus leur réputation à faire. Cependant, d'autres substances interdites moins célèbres sont également utilisées dans le dopage, tel que les P-bloquants . Cette classe de médicament, principalement utilisée dans le traitement de l'hypertension et dans les troubles cardiaques, possèdent des propriétés qui sont mises à profit dans des sports en particuliers. Ils sont capable d diminuer les réponses physiologiques au stress tels que les tremblements, les palpitations, la transpiration, ... Ils sont donc des adjuvants précieux dans des sports tels que le tir à l'arc, le tennis de table, la descente et le saut à ski, le golf,... Mais ils sont également très prisés par les musiciens, lors d'auditions importantes.Pour détecter ce type de fraude, plusieurs types de tests antidopage ont été mis au point. Cependant, la lutte contre ce fléau ne cesse de se heurter à de nouvelles substances indétectables, à de nouvelles techniques qui l'emmènent dans un combat qui semble parfois perdu d'avance. Doping is a hot topic that affects professional athletes as well as amateur sportspeople. To reach the steps of the podium, the temptation to resort to illicit substances is not going to stop yet. Among the substances, the majority are derived from medicines or administered as such. Each sport requires different skills such as strength, stamina and precision. Each skill can be improved through specific doping substances. Indeed, substances such as EPO and growth hormones have been notorious for a long time. However, other less famous banned substances such as beta-blockers are also used in doping. This class of medication, which was primarily used in the treatment of hypertension and heart diseases, has shown properties which are exploited in particular sports. Indeed, beta-blockers are able to reduce physiological stress responses such as tremors, palpitations, and perspiration. They are valuable aids in sports such as archery, table tennis, downhill skiing, ski jumping and golf, to mention a few. Moreover, they are also highly prized by musicians during major auditions. To detect this type of fraud, several types of drug tests have been developed. However, the fight against this scourge continues to come up against new undetectable substances and new techniques which take it in a fight which sometimes seems to be already lost.
