Healthcare providers currently have to cope with a challenge set by our seniors. The 65 and over population is growing rapidly and represents the largest medication use. Medical but also economic consequences for both vulnerable elders and the Society result from that fact. In the context of polypharmacy in older people, healthcare professional’s sometimes have to question their treatment options. That is why medication review processes to improve the quality have been started. My final thesis first wants to identify the medication review concept among the elderly but also its added value. Then, based on theory but also on field observations, the implication of the Belgian pharmacist in such a review is described, in both hospitals and community pharmacies. Finally, in collaboration with six patients and their general practitioner and just like in Australia, a medication review, innovative for the Belgian pharmacist, is experimented and discussed. Could it be a stepping stone for tomorrow’s pharmacist in Belgium? Les professionnels de la santé doivent actuellement faire face au défi que leur lancent nos aînés. En effet, la population âgée est en pleine expansion et représente la plus grande consommation en médicaments. Des conséquences tant médicales qu’économiques pour la personne âgée vulnérable et pour la société en découlent. Dans ce contexte de polymédication, les choix thérapeutiques sont parfois amenés à être remis en question. Face à cela, des processus de révision de médication visant à en améliorer la qualité ont été entrepris. Ce mémoire tente en premier lieu de cerner ce concept de révision de la médication des personnes âgées ainsi que sa valeur ajoutée. Ensuite, sur base théorique et d’observations sur le terrain, l’implication du pharmacien belge dans de telles révisions est décrite tant dans le milieu hospitalier qu’ambulatoire. Enfin, c’est en collaboration avec six patients et leur médecin généraliste et à l’image de ce qui se fait en Australie qu’une révision de la médication, innovante pour le pharmacien d’officine en Belgique, est expérimentée et discutée. Pourrait-elle lui servir de tremplin pour demain ?

Le pharmacien d'officine : un accompagnant, un conseiller, un commerçant, un exécutant, un préparateur ? Ce qu'un pharmacien d'officine remplira comme rôles dans le futur ne peut pas se comprendre sans une analyse préalable de ses rôles actuels et passés. Ce mémoire a donc pour but de retracer et d'expliquer cette évolution. Afin de bien saisir les enjeux futurs, notamment à travers l'exemple des produits de régime, nous ne pouvions faire l'impasse sur des personnages importants tels que Sumer, Hippocrate, Dioscoride ou Galien et sur des apports aussi précieux que ceux des Moines ou ceux du Monde Arabe. Dans le cadre de cette évolution, ce mémoire tente également de mettre en évidence les prémices de la distinction entre la profession du médecin et celle de pharmacien ; distinction qui a, elle-même, évolué et sans laquelle notre compréhension de la problématique serait dénuée de sens. Le point de vue de quatre pharmaciens, retraités ou non, nous apporte du relief et permet la construction d'un pont, d'une part, entre la théorie historique pure et l'exemple choisi des produits de régime et, d'autre part, entre cette théorie et la situation à l'étranger. À travers cette présentation historique, les commentaires des (anciens) pharmaciens d'officine et la mise en exergue de la problématique des produits de régime, ce travail tente donc d'expliquer la mutation du rôle du pharmacien et de présenter les perspectives d'avenir. A pharmacist: an accompanying person, an adviser, a businessman, an executer, an assistant? The parts that a pharmacist will play in the future cannot be understood without a previous analyse on his present and past roles. This thesis has the purpose of retrace and explain that evolution. To properly distinguish the futures stakes, especially through the example of the diet products, the only way to proceed is to talk about important personalities such as Sumer, Hippocrate, Dioscoride, or Galien, and about precious knowledges such the ones from Moines or the ones from the Arabic World. Within the framework of this evolution, this thesis a is also trying to put into light the genesis of the distinction between the profession of doctor and the one of pharmacist; distinction that has, itself, been changing over the years and that is essential to our comprehension of the phenomenon. The point of view of four pharmacists, retires or not, allowed us to accentuate and understand more, but also allowed us to link, on one hand, the classical historical theory and the chosen example of diet products, and on the other hand, this theory and the situation about the pharmacist’s position abroad. Through this historical presentation, the commentaries of the (non-active) pharmacists and the highlight of the diet products problematic, this work is trying to explain the evolution of the pharmacist’s role and it also trying to present the perspectives for the future.
