Rheumatoid arthritis is classified as auto-immune disease. Chronic inflammatory arthritis is most common in adults, occurring primarily in women from the age of 40. A better knowledge of pathophysiological mechanisms has helped in recent years to develop targeted therapies and biologic therapies. This includes monoclonal antibodies directed against TNF, a cytokine that plays a key pathogenetic role in regulating the process of inflammation, synovial proliferation and destruction of cartilage. Thus, obtaining a state of clinical remission, has now become the goal of treatment of this disease. Adalimumab, the active ingredient in Humira®, is a monoclonal humanised anti-TNF administered by subcutaneous injection at a dosis of 40 mg every two weeks. It has proven its effectiveness in terms of endpoint. However, the occurrence of side effects, mainly severe infections and reactivation of TB, requires the use with caution. Finally, advances in multidisciplinary management of RA are of great interest to improve the quality of life of patients with this chronic disease Classée parmi les maladies auto-immunes, la polyarthrite rhumatoïde est le rhumatisme inflammatoire chronique le plus fréquent de l’adulte qui survient surtout chez la femme à partir de l’âge de 40 ans. Une meilleure connaissance des mécanismes physiopathologiques a permis au cours des dernières années, le développement de traitements ciblés ou biothérapies dont les anticorps monoclonaux dirigés contre le TNFα, cytokine qui joue un rôle pathogénique clé sur la réglementation du processus d’inflammation, de prolifération synoviale et de destruction du cartilage. Ainsi, l’obtention d’un état de rémission clinique est devenue actuellement l’objectif du traitement de cette maladie. L’adalimumab, principe actif de Humira®, est un anticorps monoclonal anti-TNFα v entièrement humanisé qui s’administre par voie sous-cutanée à raison de 40 mg une fois toute les 2 semaines. Il a fait la preuve de son efficacité en terme de critères d’évaluation d’efficacité. Cependant, la survenue d’effets indésirables, principalement les infections graves et la réactivation de la tuberculose nécessite de l’utiliser avec précaution. Enfin, les progrès de la prise en charge pluridisciplinaire de la PR sont de grand intérêt afin d’améliorer la qualité de vie des patients atteints de cette maladie chronique.

Drug metabolism/pharmacokinetics and drug interaction studies have been extensively carried out in order to secure the druggability and safety of new chemical entities throughout the development of new drugs. Recently, drug metabolism and transport by phase II drug metabolizing enzymes and drug transporters, respectively, as well as phase I drug metabolizing enzymes, have been studied. A combination of biochemical advances in the function and regulation of drug metabolizing enzymes and automated analytical technologies are revolutionizing drug metabolism research. There are also potential drug–drug interactions with co-administered drugs due to inhibition and/or induction of drug metabolic enzymes and drug transporters. In addition, drug interaction studies have been actively performed to develop substrate cocktails that do not interfere with each other and a simultaneous analytical method of substrate drugs and their metabolites using a tandem mass spectrometer. This Special Issue has the aim of highlighting current progress in drug metabolism/pharmacokinetics, drug interactions, and bioanalysis.

Dear Colleagues, The rapidly changing field of medicine and healthcare is increasingly adopting scientific and technological innovations, making interdisciplinary collaborations especially important. In this context, medical disciplines are becoming increasingly interlinked with other specialities and fields. A more interdisciplinary approach to the patient is needed, especially for complex patients with numerous comorbidities, most of whom are usually elderly and fragile. The greatest challenges to human health lie at the intersection of different medical fields. An interdisciplinary medical team is increasingly necessary with the rapid expansion of medical knowledge. Given the importance of interdisciplinarity in the field of medicine and research, the international journal Medicina-Lithuania has launched this Special Issue. The Special Issue has attracted the interest of different groups of researchers, and very interesting articles from different countries. Reviews or original articles dealing with interdisciplinary medical problems, as well as articles providing an up-to-date overview of the diagnostic protocols and treatments for patients with multiple comorbidities have been published. Arthritis has a high prevalence globally and includes over 100 different types, the most common of which are rheumatoid arthritis, osteoarthritis, psoriatic arthritis, and inflammatory arthritis. The exact etiology of arthritis remains unclear and no cure exists. Anti-inflammatory drugs are commonly used in the treatment of arthritis but are associated with significant side effects. Novel modes of therapy and additional prognostic biomarkers are urgently needed for arthritis patients. This book summarizes and discusses the global picture of the current understanding of arthritis.
