L'usage qui est fait du terme borderline par les professionnels de la santé mentale et dans le langage courant recouvre un champ de plus en plus vaste. Selon Luigi Cancrini, ceci s'explique par le fait que les situations de souffrance liées à un fonctionnement mental borderline occupent un espace bien plus large (océan) que celui des névroses et des psychoses (les continents que l'océan sépare). Ce livre se penche d'abord sur les différentes significations qui ont été attribuées au terme borderline au fil du temps et souligne combien il est nécessaire de le relier à une modalité spécifique du fonctionnement de l'esprit, à laquelle nous avons tous accès et que nous devons toujours considérer comme réversible.
