Cardiac valve diseases are a major cause of morbidity and mortality around the globe. In third world countries, inflammatory rheumatic valve disease in the young represents an endemic calamity, while in Western countries, degenerative valve diseases like senile calcific aortic stenosis and mitral valve prolapse as a result of mucoid degeneration with incompetence are major causes of death in adults/the elderly. Since the 1960s, surgical valve replacement has been the only treatment option, requiring sternotomy, cardiopulmonary bypass and cardiac arrest. Permanent anticoagulation for mechanical valves and the limited durability of biological valves constituted significant drawbacks; moreover, these surgeries often requires surgical replacement. The cardiac registry at the University of Padua, Italy, has gathered more than twelve hundred cases of failed prosthetic valves, offering a unique resource for teaching and research. Conservative valve repair using minimally invasive technique showed that replacement of the native valve with sternotomy can be spared. Moreover, in the last decade venous or arterial approaches like TAVI were developed. This book covers all aspects (biological, pathological, genetic, clinical and therapeutical) of arrhythmogenic right ventricular cardiomyopathy/dysplasia, a recent cardiomyopathy which represents a very high risk of sudden death in the young and in athletes. The monograph presents the results of 5-year research program on ARVC/D supported by grants of both the European Commission and the NIH, which enabled the discovery of seven disease-causing genes, thus opening new avenues for the early identification of affected patients and for sudden death prevention. This text atlas focuses on the pathology and molecular genetics of sudden cardiac death in the young and in athletes, presenting the state of the art in the field as the basis for development and implementation of more effective prevention strategies, including, ultimately, molecular therapy that will cure the underlying biological defect. A wealth of high-resolution color images, accompanied by clear supporting text, are presented to document the anatomic pathology of the cardiac diseases most frequently responsible for sudden cardiac death in this population, including coronary artery diseases, cardiomyopathies, myocarditis, valve diseases, conduction system abnormalities, congenital heart diseases, and ion channel diseases. The role of the molecular autopsy in overcoming the limitations of morphological investigations and offering new insights and avenues for prevention is explained. The approach is, however, interdisciplinary, with close attention also to epidemiologic and clinical aspects. The authors draw throughout on their experience gained over 30 years in the course of a prospective study carried out in the Veneto Region, North East Italy. This text atlas will be of great value not only for cardiologists but also for geneticists, sports physicians, and residents in cardiology and pathology.
