Abstract

In 2010, 15 percent of Americans older than age 70 had dementia. By 2050, the number of new dementia cases among those 65 and older is expected to double. This blueprint outlines policy options to help decisionmakers improve dementia long-term services and supports (LTSS) by promoting earlier detection, improving access to LTSS, promoting person- and caregiver-centered care, supporting caregivers, and reducing dementia LTSS costs.

Abstract

Elder abuse or mistreatment (EM) — which includes psychological, physical, and sexual abuse; neglect; and financial exploitation of older adults — is a widespread problem that can have devastating consequences. People subjected to abuse or mistreatment are more likely to experience depression, cognitive decline, reduced quality of life, and premature mortality. Individuals experiencing EM often need a variety of interventions to restore health, recover from trauma, resolve or recoup financial losses, separate from their abusers, and relocate to new housing. This report presents an initial evaluation of the nation's first elder-specific shelter — the Harry and Jeanette Weinberg Center for Elder Justice, which is part of the Hebrew Home at Riverdale. Located in the Bronx, New York, the Hebrew Home is a nonprofit residential health care facility with 560 beds, part of a continuum of care community that provides a full spectrum of health care, home care, and housing on a nonsectarian basis. The authors seek to shed light on the Weinberg Center model, which aims to benefit the growing number of individuals experiencing EM and also may have wider societal benefits. Specifically, the authors describe the Weinberg Center's shelter model, examine some of the more important outcomes for Weinberg Center clients, and begin to quantify some of the costs and benefits, including potential cost savings, of the model for individuals experiencing EM, public payers, and society. However, it is important to note that this is an initial evaluation with a limited scope. A full evaluation would employ a rigorous experimental design.

Abstract

The U.S. Constitution mandates that the federal government count all persons living in the United States every ten years. The census is critical to states because its results are used to reapportion seats in the U.S. House of Representatives; guide redistricting; and form the basis for allocating federal funds, such as those used for schools, health services, child care, highways, and emergency services. In response to long-standing concerns about the accuracy of census data and about a possible undercount, a group of researchers conducted the California Neighborhoods Count (CNC) - the first-ever independent, survey-based enumeration to directly evaluate the accuracy of the U.S. Census Bureau's population totals for a subset of California census blocks. This 2020 research was intended to produce parallel estimates of the 2020 Census population and housing unit totals at the census block level, employing the same items as the census and using enhanced data collection strategies and exploration of imputation methods. Although the CNC was intended to largely replicate census data collection processes, there were a few methodological differences: For example, much of the address canvassing for the 2020 Census was done in-office, whereas the CNC team undertook a complete in-person address-listing operation that included interviews with residents and door-to-door verification of each structure. In this report, the researchers detail their methodology and present the enumeration results. They compare the 2020 Census counts with the CNC estimates, describe limitations of their data collection effort, and offer considerations for future data collection.

Abstract

The broad goals of New York State's Medicaid Section 1115 Waiver are to enroll a majority of Medicaid beneficiaries into managed care, increase access and service quality, and expand coverage to more low-income New Yorkers. The RAND Corporation was competitively selected as the independent evaluator to assess two components under this 1115 Demonstration Waiver: the Managed Long-Term Care (MLTC) program and the 12-month continuous eligibility policy, which guarantees enrollees Medicaid coverage regardless of changes in income in the 12 months after eligibility determination and enrollment. This final interim evaluation report examines whether these two components have helped achieve the program's goals. The RAND team's analyses show that the Demonstration has expanded access to managed care through mandatory MLTC enrollment and 12-month continuous eligibility. The team found no evidence of a significant change in patient safety or quality of care. The authors note that, although this means that there is no evidence the Demonstration achieved the goal of improving quality of care, increasing access without compromising quality of care is a success in its own right.
