Experiences of Health Centers in Implementing Telehealth Visits for Underserved Patients During the COVID-19 Pandemic: Results from the Connected Care Accelerator Initiative

Abstract

In early 2020, as the coronavirus disease 2019 (COVID-19) pandemic emerged, widespread social-distancing efforts suspended much of the delivery of nonurgent health care. Telehealth proved to be a viable alternative to in-person care, at least on a temporary basis, and utilization skyrocketed. Many Federally Qualified Health Centers (FQHCs) serving low-income patients started delivering telehealth visits in high volume in March 2020 to help maintain access to care. This sudden and dramatic change in health care delivery posed numerous challenges. Health centers had to quickly make changes to technology, workflows, and staffing to accommodate telehealth visits. To support health centers in these efforts, the California Health Care Foundation established the Connected Care Accelerator (CCA) program, a quality improvement initiative that was launched in July 2020. RAND researchers evaluated the progress of FQHCs that participated in the CCA initiative by investigating changes in telehealth utilization and health center staff experiences with implementation. In this report, researchers review recent literature on telehealth implementation in safety net settings. They also present new information on the experiences of the 45 CCA health centers, drawing from data on visit trends, interviews with health center leaders, and surveys of health center providers and staff. Telehealth has the potential to increase access to care and deliver care that is more convenient and patient-centered; however, ongoing research is needed to ensure that telehealth is implemented in a way that ensures high-quality care and health equity.

Abstract

To help schools safely return to in-person instruction and expand coronavirus disease 2019 (COVID-19) testing of teachers, other staff, and students, it will be imperative to successfully scale up and spread promising practices from K–12 schools and districts that were early adopters of COVID-19 testing in the fall semester of 2020. This report describes approaches that early adopters of COVID-19 testing in U.S. K–12 schools were using as of December 2020, highlighting key facilitators that made testing feasible, acceptable, and effective in these schools. In the first half of the report, the authors describe the landscape of testing as of December 2020, summarize the benefits and costs of early adopters' testing programs, and offer recommendations to school leaders, district officials, and policymakers for implementing feasible, acceptable, and effective COVID-19 testing in K–12 schools. They then profile COVID-19 testing programs in selected schools, districts, and states, with a focus on describing the practical details of their testing approaches, challenges they had to overcome, and key factors that facilitated their efforts.

Abstract

Between 2000 and 2021, the U.S. Department of Defense diagnosed more than 444,300 service members as experiencing at least one traumatic brain injury (TBI) during their military service. The number of TBIs experienced, and their severity, can affect the trajectory of and prognosis for recovery. Much progress has been made in developing, implementing, and disseminating effective treatments for TBI, yet gaps remain in understanding the long-term care and support needs for veterans who have sustained one or more TBIs during their military service. This report presents the findings from a study commissioned by Wounded Warrior Project (WWP) and conducted by the RAND Corporation to identify the long-term outcomes of TBI for veterans serving since the terrorist attacks of September 11, 2001; the future needs of this population; effective treatments for TBI; and the availability of community-based resources. The information in this report can be used to better understand which approaches may offer the best care for veterans with TBI, as well as help inform the care and support offered to veterans and their caregivers.
