Craniofacial Trauma, Diagnosis and Management offers detailed guidance on the diagnosis, surgical planning, and interdisciplinary treatment of craniofacial trauma. The book is divided into two parts. The first, devoted to classification and diagnosis of craniofacial fractures, includes chapters on anatomy, radiology, fracture classification, fracture mechanisms, epidemiological aspects, symptoms, and specific related aspects of neuro-craniofacial injuries. The second part addresses the treatment of craniofacial trauma, examining operative principles and providing step-by-step descriptions of a variety of hard and soft tissue reconstructive procedures. Complications and late sequelae following craniofacial reconstruction are examined, and a further chapters is devoted to delayed reconstruction of craniofacial defects. New developments and the role of computer-assisted treatment planning are discussed in the final section. The second part then focuses on treatment, explaining operative principles and providing step-by-step descriptions of a variety of hard and soft tissue reconstructive procedures. Here, individual chapters are devoted to neurosurgical management, surgical repair of fractures, methods of dural and skull base treatment, osteosynthesis, intraoperative navigation techniques and systems, reconstruction with bone grafts and alloplastic materials, and complications and late sequelae. The book concludes by discussing surgical strategy in complex craniofacial trauma care and presenting a treatment algorithm that takes into account new developments. Facial fractures, particularly those resulting from severe injuries with multiple fr- tures in the cranio-maxillo-facial region, are the most common form of neurocranial injuries (Hausamen and Schmidseder 1975; Machtens 1987; Brachvogel et al. 1991; Wahlmann and Wagner 1991). Depending on the complexity and level of the fracture, the frontobasal invol- ment of all craniofacial injuries varies between 30 and 70% (Manson et al. 1987; Raveh et al.1992; Weerda 1995; Joss et al. 2001). In addition to the challenging rec- struction of severe craniofacial injuries, specifc diagnostic, pathogenetic and the- peutic problems arise as a consequence of the accompanying frontobasal fractures. A considerable optimization in the treatment of these profound injuries can be achieved by a routine team approach of maxillofacial, neurosurgical, and anesthes- logical specialists with the appropriate diagnostic and therapeutic resources at their disposal. The interdisciplinary treatment of patients with severe craniofacial injuries is state-of-the-art and focused on trauma centers equipped with the adequate inf- structure (Reulen and Steiger 1994; Weingart et al. 1996; Seidl et al. 1998). The purpose of the present monograph is to analyze and introduce an established therapy concept for craniofacial fractures with anterior subcranial involvement, with reference to the surgical approach and the postoperative results. Of particular interest are etiological, epidemiological, and pathomechanical characteristics in neu- craniofacial injuries.

This first volume in a multi-volume series considers the gains in information and knowledge that have resulted from preoperative and postoperative 3D imaging using new radiologic protocols in maxillofacial surgery, with the corresponding consequences for the surgeon. It contrasts the established standard techniques of orthognathic oral and maxillofacial surgery with new considerations and insights based on years of experience and analysis of clinical activity in this subspecialty of oral and maxillofacial surgery. The book is deliberately structured so that the clinical situation is juxtaposed with a graphic representation for better understanding, which is intended to point out special situations that can in turn positively influence the surgical planning and execution of the procedure in order to avoid undesirable results in individual cases. Graphic representations illustrate the three-dimensionality of the complex splitting technique of the mandible where text or radiographs are insufficient. A corresponding book that combines clinical anatomical situations with pre- and postoperative preparation and clear explanation does not yet exist. It is aimed at a wide readership of students and professionals in oral and maxillofacial surgery, orthodontics, plastic and craniofacial surgery, and otolaryngology.
