Abstract

Sciensano, het federale instituut voor epidemiologisch onderzoek, lanceert het Health Status Report. Nieuw aan het rapport is voorlopig dat het verschillende bronnen voor gezondheidsstatistiek samenbrengt. Het Health Status Report combineert de resultaten van verschillende bronnen om in te schatten hoe gezond de Belg is. Algemeen genomen is de Belg eerder gezond, maar op sommige vlakken scoren we niet echt goed. Daarnaast zijn er belangrijke ongelijkheden, zowel op regionaal als op socio-economisch vlak. Het onlinerapport is gebaseerd op de meest recente gegevens en wordt constant bijgewerkt met resultaten van nieuwe studies, zodat gezondheidsbeleidsmakers op continue basis beleid kunnen ontwikkelen op maat van de actuele behoeften.

Abstract

Giardiasis is one of the most common gastrointestinal parasitic diseases worldwide and it is caused by an enteric pathogen named Giardia spp. Giardiasis is the most commonly reported human parasite of gastroenteritis with frequent sporadic cases and community-wide outbreaks via water- and -in smaller proportion- foodborne pathways. Giardiasis encompasses a complex and diverse spectrum of symptom patterns ranging from asymptomatic carriage to acute and chronic health states. Prolonged chronic infections eau enhance extra-intestinal and post-infectious sequelae. Estimation of the population health status, also known as "disease burden", requires well-defined information on clinical outcomes of infection. Unfortunately, regarding the diverse symptomatology and limited knowledge on pathogenesis mechanisms, giardiasis burden is often difficult to estimate and report. Objectives: To accurately estimate the disease burden of giardiasis and explain satisfactory its clinical impact, it is therefore necessary to have information on the nature and frequency of these sequelae. To this end, we aim to be able to get a better understanding of the symptoms of Giardia infection in humans. The aim of this thesis will be to review the clinical impact of giardiasis worldwide using a systematic search approach. To this end we will proceed in a two-step approach as follows: 1.First we will review existing assessments of the disease burden of Giardia. This will give an overview of the current disease models (i.e., lists of giardiasis symptoms .2.Next we will perform a systematic review of clinical symptoms associated with Giardia infection in humans in order to identify the health states which are associated with the giardiasis burden and estimate the occurrence of potential sequelae (acute and chronic) of giardiasis. Methods: In order to address the identified gaps, we chose to perform systematic review of the literature according to PRISMA guidelines. We will focus on prospective studies (i.e., cohort studies, longitudinal studies and outbreak investigations) retrospective studies (i.e., case-control studies) and cross-sectional studies. Findings: To date, we have irrefutable evidence of variability in clinical outcomes associated with Giardia infection. Typically assumed as a re-emerging and neglected disease, giardiasis places a heavy burden in both developed and developing countries. It has been linked to persistent diarrhoea that may contribute to substantial weight loss, retardation of growth and development in infants, cognitive impairment and malabsorption. In addition, it has been confirmed that acute giardiasis is associated with significant post-infectious chronic sequelae in adults such as imitable bowel syndrome, chronic fatigue or functional dyspepsia. These persistent sequelae affect the quality of life by continuing to inflict on patients long lasting discomfort and pain.

Abstract

Le but de ce mémoire est de décrire la malnutrition dans une maison de repos et de soins.3 objectifs sont fixés dans le cadre de ce mémoire. Le premier objectif sera d'établir la faisabilité et la fréquence du diagnostic de dénutrition, en fonction du MNA-SF (mini-nutrional assessment, short form). Le second sera d'investiguer les relations qui existent entre l'état nutritionnel et la dépendance fonctionnelle (selon l'échelle de Katz comprenant 6 domaines) et la qualité de vie (EQ-5D-5L).Enfin, le troisième sera de proposer des pistes de prise en charge nutritionnelle qui intègrent ces problématiques. Pour ce faire une étude transversale prospective a été réalisée au début de l'année 2017. L'échantillon reprenait 131 personnes vivant à la résidence avec une moyenne d'âge de 80 ans. La classification de l'échantillon en fonction de l'état nutritionnel s'est faite sur base de la version courte du MNA. Cette étude a confirmé la prévalence élevée des problèmes nutritionnels chez les résidents : 38% étaient en dénutrition (MNA-sf <7) et 46% à risque de dénutrition (MNA-sf entre 8-11). Seulement 16% présentaient un bilan nutritionnel satisfaisant (MNA-sf entre 12-14).Cette étude a permis de classifier la population sur base de l'échelle de Katz, échelle d'évaluation de l'autonomie fonctionnelle chez le résident. Elle révèle que 31% de notre échantillon est dépendant physiquement et psychiquement (catégorie Cd) et seulement 13% sont indépendant physiquement et psychiquement (catégorie A). Notre étude révèle que 75% des résidents en catégorie Cd sont dénutris. Selon une analyse multivariée; il s'avère que la catégorie Cd est significativement associée à une dénutrition et que les paramètres «se laver », « s'habiller » et« aller au toilette » sont significativement associés aux résidents dénutris. Les résidents ont été par ailleurs soumis à un autre test ; il s'agit du questionnaire EuroQoL 5D. Ce questionnaire permet d'évaluer la qualité de vie selon 5 dimensions. La qualité de vie des dénutris est significativement inférieure à la qualité de vie du groupe ayant un statut normal ou à risque de dénutrition. Nous constatons que la dénutrition dans notre échantillon est liée principalement au fait que les personnes ait une motricité faible mais aussi au fait qu'ils aient des problèmes neurologiques. En effet, le fait d'être dément, désorienté implique un gros risque de dénutrition. Dès lors, il importe que tous le personnel soignant dans cette maison de repos soient sensibilisé à la fréquence de la dénutrition. Son traitement nécessite une vision multidimensionnelle et une collaboration pluridisciplinaire.

Abstract

Quantitative microbial risk assessment (QMRA) aims to quantify the human health effects resulting from exposure to microbiological hazards. QMRA is one component of risk analysis, together with risk management and risk communication. QMRA itself is classically divided into four components, i.e., hazard identification, exposure assessment, dose-response assessment and risk characterisation. Driven by guidelines and regulations, QMRA is increasingly applied to safeguard food safety and water quality. Although QMRA is a rapidly evolving scientific field, only few dedicated software tools are available. To address this gap, we developed QMRA, an R package that provides a comprehensive suite of functions for performing exposure assessment, dose-response assessment and risk characterisation in the context of microbial risk assessment. Different parametric models are available and can be fitted by both maximum likelihood and Bayesian methods. The different functionalities are exemplified via a range of built-in exposure and dose-response datasets. Work is in progress to further extend the available functionalities. We hope that the QMRA package may prove to be a useful tool for researchers involved in microbial risk assessment.

Abstract

Health innovations often meet needs which are more closely linked to the supply aspect (industry, hospitals, care providers, etc.) than to patient demand. The public authorities are aware of this fact and are trying to reverse this tendency. With this is mind, KCE has developed a method for identifying in a scientific and structural way the most important patient needs that are not being met. This method could be used in NIHDI’s reimbursement decisions, and it could also help, firstly, to provide better information to health professionals on actual patient needs and, secondly, to determine the research priorities to be set in the agendas of industry and of the bodies that finance public research.
