The U.S. military's current concept of operations for expeditionary medical care emphasizes quickly moving patients to a series of successively more sophisticated medical facilities that provide the patients with care necessary to treat their injury or condition. Balancing the deployment of treatment and evacuation resources is necessary to ensure that the right mix of resources is available in a timely fashion. The authors propose a planning concept that, by promoting patient flow rate as the common unit of measurement, will help integrate medical planning across treatment and evacuation functions, across the increasing levels of care, and across the different military services. The primary medical planning tool approved for use by combatant commands for developing their operational plans, the Joint Medical Analysis Tool (JMAT), already operates in a manner consistent with this concept. However, modifications and enhancements to JMAT are necessary to ensure that the concept can be fully implemented.

An analysis of combat support experiences associated with Operation Enduring Freedom that compares these experiences with those associated with Operation Allied Force (OAF). Its objectives were to indicate the performance of U.S. Air Force combat support in OEF, examine how Agile Combat Support concepts were implemented in OEF and compare OAF and OEF experiences to determine similarities and applicability of lessons across experiences, and to determine whether some experiences are unique to particular scenarios.

The Defense Logistics Agency (DLA) supplies common military items to the armed services and other organizations while seeking to achieve customer service goals and minimize cost. When demand for an item rises unexpectedly, providing effective customer service is challenging, and when demand for an item falls unexpectedly, DLA can be left with the sunk cost of excess inventory that it then disposes. The more quickly and efficiently DLA can respond to changes in demand - that is, the more agile the DLA supply chain - the more it can minimize such impacts. This report examines DLA supply chain agility and ways to improve it. Specifically, it focuses on the value of and the approaches DLA could take for reducing lead times, optimizing order quantities, and improving information flow from customers. The overarching recommendation is to increase enterprisewide emphasis on supply chain agility, with involvement from the most-senior management levels across the DoD supply chain management enterprise and flowing downward to all levels. Continuous attention to supply chain agility should become part of the supply chain DNA. More-specific business practice recommendations are also provided in the report.

The government of Puerto Rico developed a plan to recover from the destruction caused by Hurricanes Irma and Maria, build resilience to withstand future disasters, and restore the struggling economy. The Homeland Security Operational Analysis Center (HSOAC), operated by RAND Corporation under contract with the U.S. Department of Homeland Security, worked with the Federal Emergency Management Agency (FEMA) to assist with the development of the plan. Acting in support of FEMA's Community Planning and Capacity Building (CPCB) sector, HSOAC conducted surveys, interviews, and focus groups with municipalities, government leaders, subject-matter experts (with expertise in law enforcement, emergency management, community planning, etc.), nonprofits, and community residents, among others, to provide critical community context for CPCB efforts. HSOAC also used multiple data sources to estimate the outmigration of residents who in the wake of the hurricane moved out of their communities to the continental United States. The authors describe the status of Puerto Rico's communities in terms of population characteristics and dynamics, community and individual preparedness, and economic pressures leading up to the landfalls of the hurricanes. They also report on the impact that the hurricanes had on Puerto Rico's communities in terms of damage, community stress, and migration away from the island. These analyses and discussions informed the development of 15 courses of action (COAs) aimed at improving Puerto Rico's capacity for emergency preparedness, coordination, communication, recovery planning, and research and training, to meet the needs of Puerto Rico and its vulnerable communities during a disaster.
