Abstract

In this edited volume, a diverse group of scholars present and discuss challenging cases in the field of paediatric research ethics. After years of debate and controversy, fundamental questions about the morality of paediatric research persist: is it ever permissible to use a child as a means to an end? How much authority should parents have over decisions about research that involves young children? What should be the role of the older child in decisions about research participation? How do the dynamics of hope and desperation influence decisions about research involving dying children? Should children or their parents be paid for participation in research? What about economic incentives for doctors, researchers and the pharmaceutical industry? Most importantly, how can the twin goals of access to the benefits of clinical research and protection from research risk be reconciled? Following an introductory overview by editor Eric David Kodish, the book is divided into three sections of case studies: Research Involving Healthy Children, Research Involving At Risk Children, and Research Involving Children with Serious Illness. Abstract

Pediatric Surgery Digest offers a praxis-oriented and up-to-date overview of all subjects in pediatric surgery according to the European surgical curriculum. Additionally, interdisciplinary aspects concerning pediatrics, adult surgery, obstetrics and other disciplines working with children were considered. Abstract

Progress in genetic and reproductive technology now offers us the possibility of choosing what kinds of children we do and don't have. Should we welcome this power, or should we fear its implications? There is no ethical question more urgent than this: we may be at a turning-point in the history of humanity. The renowned moral philosopher and best-selling author Jonathan Glover shows us how we might try to answer this question, and other provoking and disturbing questions to which it leads. Surely parents owe it to their children to give them the best life they can? Increasingly we are able to reduce the number of babies born with disabilities and disorders. But there is a powerful new challenge to conventional thinking about the desirability of doing so: this comes from the voices of those who have these conditions. They call into question the very definition of disability. How do we justify trying to avoid bringing people like them into being? In 2002 a deaf couple used sperm donated by a friend with hereditary deafness to have a deaf baby: they took the view that deafness is not a disability, but a difference. Abstract

Pediatric Bone Sarcomas: Epiphysiolysis before excision addresses malignant bone tumours by focusing upon physeal distraction (epiphysiolysis) before excision. This technique has long been used by the authors of this book in preserving the joint in metaphyseal bone tumours in children. They are subsequently now sharing their knowledge and experience with all those in the field of orthopedic surgery. In the last 30 years, the outcome for patients affected by malignant bone tumours has improved dramatically. Better knowledge of the disease, improved imaging methods, new surgical techniques and particularly the advent of chemotherapy have brought about increasing success in this field. Preserving the life of these patients 30 years ago was the only aim of physicians. Now, survival rates have increased and the new challenges posed to the surgeon are on preserving the limb and its function, whilst avoiding complications. 30 years ago joint preservation close to the bone tumour was unheard of. More recently, this is a desire and possible outcome common to everyone who treats these patients. Targeted primarily towards orthopedic surgeons and pediatric oncologists this book will also be of interest to all those in the fields of orthopedics, oncology, pediatrics and surgery.
