Nous avons étudié de manière approfondie la réponse anti-tumorale médiée par les lymphocytes T cytolytiques (CTL : Cytolic T Lymphocytes) chez un patient porteur d’un mélanome.
Des cellules mononuclées du sang périphérique ont été stimulées in vitro par des cellule tumorales autologues dont une culture stable avait pu être établie. Suite à cette culture de masse, les lymphocytes T ont été clonés et plusieurs clones CTL lysant spécifiquement la tumeur ont été obtenus. Quelques-uns de ces clones ont été utilisés lors d’immunosélections des cellules tumorales en vue d’obtenir des variants tumoraux de perte d’antigène. Les variants obtenus permirent la mise en évidence de quatre antigènes différents, reconnus par des CTL à la surface des cellules tumorales.
Nous avons ensuite utilisé une technique de dilution limite des lymphocytes circulants pour évaluer la fréquence des CTL anti-tumoraux dans le sang. Chez ce malade, cette fréquence est très élevée : environ un précurseur de CTL spécifique anti-tumoral pour 1000 cellules mononuclées. Au moyen de différents variants tumoraux de perte d’antigène, nous avons observé que les précurseurs de CTL dirigés contre l’un des quatre CTL anti-tumoraux.
Face à cette prédominance ‘un type de CTL, nous avons analysé le répertoire lymphocytaire dirigé contre l’antigène dominant. Pour ce faire, la séquence de la chaîne ß du récepteur T d’une série de clones CTL spécifiques de l’antigène a été déterminée. Cette analyse génétique a révélé que tous les clones CTL dirigés contre l’antigène dominant possèdent la même chaîne ß. Ils sont donc issus d’un même précurseur qui s’est fortement multiplié in vivo.
