Abstract

The search for the most favorable therapeutic ratio – at which ablation of cancer is achieved while normal tissues are conserved – has been modern radiation oncology’s equivalent of the quest for the Holy Grail. Our awareness of the late effects of radiation grew during the past century as new modalities were introduced. Heightened normal tissue reactions accompanied the higher rates of cancer ablation achieved by escalation of radiation doses, accelerated fractionated radiotherapy, and aggressive concurrent chemotherapy and radiation regimens. This volume is based on the LENT V NCI-sponsored meeting held in May 2004 and the CURED I conference held in 2006. Written by experts in the field, it addresses a number of critical topics relating to late effects, such as mechanisms of injury, the role of screening, options for interventions, second malignancies, and prevention. It is hoped that it will assist the reader in understanding how to prevent and treat the long-term side-effects of irradiation.

Abstract

Dit boek werd gepubliceerd in 1996 op initiatief van NIRAS. Het is een synthese van de kennis over de gevolgen van lage dosissen ioniserende straling. Het is in de eerste plaats bestemd voor personen die niet dagelijks met het onderwerp te maken hebben maar belangstelling hebben voor wetenschappen ; het is ook bestemd voor personen die beroepshalve met een deelaspect van het onderwerp bezig zijn en een meer algemeen overzicht willen hebben. In de diverse hoofdstukken wordt getracht een antwoord te geven op de volgende vragen : wat weten we met zekerheid? Over welke punten bestaat er nog onzekerheid? Wat doet de overheid om ons tegen straling te beschermen? Verschillende deskundigen hebben de hoofdstukken geschreven die te maken hebben met hun vakgebied. De redactie van het boek werd gevolgd en geleid door een wetenschappelijk comité. Er werd veelvuldig gebruik gemaakt van illustraties, grafieken en tabellen, die bijdragen tot de uitzonderlijke waarde van het boek. Tevens werd een glossarium opgesteld van de belangrijkste wetenschappelijke termen en werd aan het einde van elk hoofdstuk een lijst met referentiewerken toegevoegd. Dit boek heeft de ambitie de lezer duidelijke en objectieve informatie te verstrekken over een onderwerp dat ons allen aangaat en betrekking heeft op maatschappelijke problemen, maar dat vaak ook passionele controverses doet rijzen die veelal een weerspiegeling zijn van persoonlijke opinies of particuliere belangen: de gezondheidseffecten van lage dosissen ioniserende straling.

Abstract

This book covers all aspects of chronic radiation syndrome (CRS) based on observations in a unique sample of residents of the Techa riverside villages in the southern Urals who were exposed to radioactive contamination in the 1950s owing to releases of liquid radioactive wastes from Mayak Production Association, which produced plutonium for weapons. In total, 940 cases of CRS were diagnosed in this population and these patients were subjected to detailed analysis. The opening chapters address the definition and classification of CRS, epidemiology and pathogenesis, covering molecular and cellular mechanisms, radioadaptation, and the role of tissue reactions. The pathoanatomy of CRS during the development and recovery stages is discussed for all organ systems. Clinical manifestations of CRS at the different stages are then described in detail and the dynamics of hematopoietic changes are thoroughly examined. In the following chapters, principles of diagnosis (including assessment of the exposure doses to critical organs) and differential diagnosis from a wide range of other conditions are discussed and current and potential treatment options, described. The medical and social rehabilitation of persons with CRS is also covered. This book, which casts new light on the condition, will be of value for all practitioners and researchers with an interest in CRS.
