For over 2000 years, vulval sex fluids were understood to be a natural part of female pleasure, only to become disputed or categorically erased in the twentieth century. Today what do we really know about female ejaculation and squirting? What does the research show, and why are so many details unknown? In Juice, Stephanie Haerdle investigates the cultural history of female genital effluence across the globe and searches for answers as to why female ejaculation—which, according to some reports, is experienced by up to 69 percent of all women and those who have vulvas upon climaxing—has been banished to the margins as just another male sex fantasy. Haerdle charts female juices from the earliest explanations in the erotic writings of China and India, to interpretations of the fluids by physicians, philosophers, and poets in the Middle Ages and early modern period, to their denial, contestation, and suppression in late nineteenth-century Europe. Contexte : L’éjaculation précoce est un trouble assez rependu touchant un homme sur trois. Les techniques de prise en charge de ce problème ne sont pas bien connues par les professionnels de la santé. Les patients ne savent pas toujours vers qui se tourner pour trouver de l’aide ou sont gênés et n’osent pas en parler. Il est nécessaire de leur proposer une information claire et précise afin de les aider ou de les orienter au mieux. Pour ce faire, la bibliothérapie est une bonne solution. Mais cette thérapie peut-elle être améliorée par un accompagnement de faible intensité ? Méthode : 150 sujets atteints d’éjaculation précoce et 27 partenaires ont participé à l’étude. Ils devaient remplir des questionnaires dans une phase de pré-test et de post-test évaluant le fonctionnement sexuel ainsi que diverses mesures cliniques. Les sujets étaient séparés en deux groupes, le premier disposant de la bibliothérapie seule ; le second disposant de la bibliothérapie couplée à un accompagnement clinique de faible intensité. Le but est d’observer si l’accompagnement thérapeutique influence le résultat de la bibliothérapie. Résultats : l’accompagnement semble être bénéfique pour les sujets de l’étude mais les effets ne sont pas très marqués. L’accompagnement améliore la durée de pénétration ainsi que la satisfaction sexuelle globale. De plus, un bon accompagnement est lié avec un bon sentiment de changement chez les sujets. Conclusion : l’accompagnement clinique offre une plus-value à la bibliothérapie sur le fonctionnement sexuel mais pas sur le profil psychologique plus général du sujet. Nous pensons que l’accompagnement peut donner un meilleur effet si les séances sont plus nombreuses et que le sujet et le thérapeute disposent de plus de temps pour élaborer une technique de traitement personnalisée.

Sexuality is considered as a great human value related to happiness and satisfaction, but unfortunately, when affecting mental disorders, they tend to be associated with second level human functions. Nevertheless, sexual dysfunction often accompanies psychiatric disorder, intensely influencing compliance, quality of life and human relationships. Sexuality could be influenced either by a mental disorder itself, difficulties to get and maintain couple relationships or by the use of psychotropic treatments. Treatment-related adverse events are unfortunately under-recognized by clinicians, scarcely spontaneously communicated by patients, and rarely investigated in clinical trials. The most frequent psychotropic compounds that could deteriorate sexuality and quality of life include antidepressants, antipsychotics and mood regulators. There are important differences between them related to some variations in mechanisms of action including serotonin, dopamine and prolactin levels. Little is known about the relevance of sexuality and its dysfunctions in chronic and frequent mental and neurological disorders, such as psychosis, mood disorders, anxiety, phobias, eating disorders, alcohol or drug dependencies, epilepsy and childhood pathology. Poor sexual life, low satisfaction and more frequent risky sex behavior than in the general population are associated with severe mental diseases. There is a need for increasing research in this field, including epidemiological, psychological, neurophysiological, neuroanatomical and genetic variables related to sexual life to get a better understanding of the implicated mechanisms. To increase the sensibility of clinicians, the identification and management of sexual disturbances after the onset of any mental disorder should be highlighted. This would avoid unnecessary suffering and deterioration of quality of life.
