L'utilisation des cellules souches mésenchymateuses gagne du terrain en médecine vétérinaire dans le domaine de la médecine régénérative, notamment chez le chien et le cheval pour le traitement de pathologies articulaires inflammatoires dégénératives, comme l'ostéoarthrose, pour lesquelles le vétérinaire ne dispose pas actuellement de solution curative efficace dans son arsenal thérapeutique. Cependant le manque de recul sur l'efficacité de l'injection de ces cellules dans un micro-environnement soumis à l'inflammation pourrait mener à leur emploi inutile, voire délétère. Nous nous sommes donc demandé quel était l'état des lieux des connaissances sur le comportement des cellules souches mésenchymateuses dans un milieu articulaire soumis à l'inflammation. Si in-vitro l'exposition aux cytokines inflammatoires du liquide synovial module leurs sécrétions paracrines et l'expression des marqueurs membranaires, l'injection intra-articulaire sur des modèles d'ostéoarthrite démontre leur action anti-inflammatoire et immunomodulatrice sur les cellules résidentes, leur effet chondroprotecteur, mais aussi leur faible persistance dans le milieu intra-articulaire inflammé. Au niveau des résultats cliniques, si les cellules souches mésenchymateuses en intra-articulaire sont sûres et efficaces pour réduire en partie les signes cliniques de l'ostéoarthrose, les nombreuses variations de nature et de qualité des produits cellulaires utilisés empêche actuellement une évaluation objective de l'impact réel de ce traitement.
