Abstract

Frederic C. Schaffer challenges the assumption often made by American scholars that democracy has been achieved in foreign countries when criteria such as free elections are met. Elections, he argues, often have cultural underpinnings that are invisible to outsiders. To examine grassroots understandings of democratic institutions and political concepts, Schaffer conducted fieldwork in Senegal, a mostly Islamic and agrarian country with a long history of electoral politics. Schaffer discovered that ideas of "demokaraasi" held by Wolof-speakers often reflect concerns about collective security. Many Senegalese see voting as less a matter of choosing leaders than of reinforcing community ties that may be called upon in times of crisis.By looking carefully at language, Schaffer demonstrates that institutional arrangements do not necessarily carry the same meaning in different cultural contexts. Democracy in Translation asks how social scientists should investigate the functioning of democratic institutions in cultures dissimilar from their own, and raises larger issues about the nature of democracy, the universality of democratic ideals, and the practice of cross-cultural research.

Abstract

The occurrence of multiple resistance among bacteria Gram-negative responsible for nosocomial infections in neonatal units is, at the present time, a major concern. Only few therapeutic options being available to try to control such infections. The neonatologists are forced to use older but also potentially more toxic antibiotics making their assessment already difficult, in the neonatal population. It is in this context that ciprofloxacin proved to be a necessary alternative despite the potential risk of osteoarticular and neurodevelopmental toxicity feared in newborns. This thesis first aims at describing the use of ciprofloxacin in the neonatal population. Then the challenges associated with conducting clinical studies in this population and the possible ways to overcome them are reviewed. Lastly, the data related to the dosage regimen and to the safety of ciprofloxacin for treatment of neonates suffering of nosocomial infections caused by multi-resistant Gram-negative bacteria are analyzed. Despite the few studies completed to date, a pharmacokinetic study has allowed to determine a suitable dosage regimen for newborns. However, eventhough the long term safety of ciprofloxacin has not been studied in neonates, the toxicity could not be demonstrated during (pre)-clinical studies. L’émergence de résistances multiples parmi les bactéries à Gram-négatif constitue à l’heure actuelle une préoccupation majeure dans les services de néonatalogie. Face au nombre limité d’options thérapeutiques disponibles pour contrer de telles infections, les néonatologistes sont contraints à recourir à des antibiotiques plus anciens mais potentiellement plus toxiques rendant ainsi nécessaire leur évaluation, pourtant difficile, dans la population néonatale. C’est dans ce contexte que la ciprofloxacine s’est avérée être une alternative nécessaire malgré le risque potentiel de toxicité ostéo-articulaire et neuro-développementale redouté chez les nouveau-nés. Ce travail de fin d’études tente dans un premier temps de cerner l’utilisation de la ciprofloxacine dans la population néonatale. Ensuite, les difficultés associées à la réalisation d’études cliniques et les éventuels moyens de les surmonter sont passés en revue. Finalement, les données disponibles concernant la posologie ainsi que la sécurité d’emploi de la ciprofloxacine chez les nouveau-nés atteints d’infections nosocomiales à bactéries à Gram-négatif multi-résistantes ont été analysées. Malgré le peu d’études finalisées à ce jour, l’étude pharmacocinétique a permis de déterminer une posologie adaptée aux nouveau-nés. La sécurité de la ciprofloxacine à long terme n’a pas été étudiée chez les nouveau-nés mais aucune toxicité significative n’a pu être démontrée en (pré) –clinique).
