Pelvic discontinuity is a situation which occurs increasingly in total hip arthroplasty revision . It happens in case of massive acetabular bone loss or when a fracture exists through the acetabulum and then separates the superior and the inferior aspects of the pelvis. Treatment remains complex, many surgical techniques are available and to date, no alternative appeared to be the best management. This study reviews 33 cases of pelvic discontinuity treated at the Cliniques Universitaires Saint-Luc (Brussels). Severa! techniques were used and we compared the results : allografts with plates, acetabular cages, antiprotrusio cages, trabecular metal augments and cups. Failure was assessed when pelvic discontinuity persisted and when a new revision was needed. We evaluated functional scores, checked the pre- and postoperative radiographs, scans and also recorded complications. Mean follow-up was 41 months (9 to 125). Antiprotrusio cages with massive femoral allografts and hemipelvis grafts with plates appear to be reliable techniques, at last at midterm follow-up. La discontinuité pelvienne est une situation rencontrée de manière croissante en chirurgie de révision des prothèses totales de hanche survenant en cas de perte osseuse importante au niveau de l'acétabulum ou en cas de fracture acétabulaire séparant l'hémipelvis supérieur de l'hémipelvis inférieur. Le traitement est complexe, les techniques disponibles sont nombreuses et à ce jour , il n'existe pas de solution universelle pour leur prise en charge. Dans ce travail nous avons revu une série de 33 discontinuités traitées aux Cliniques universitaires Saint-Luc (Bruxelles) par différentes techniques : allogreffes et plaques, cages acétabulaires simples, cages antiprotrusion et enfin augments et cupule en métal trabéculaire. Nous avons comparé les résu ltats. L'échec a été retenu comme la persistance de la discontinuité pelvienne et la nécessité d'une nouvelle révision. Nous avons évalué les scores fonctionnels, revu les images radiographiques et scannographiques pré- et postopératoires, et recensé les complications. La durée moyenne du suivi était de 41 mois (9 à 125). Les cages antiprotrusion associées aux allogreffes massives de fémur ainsi que les allogreffes d'hémi-bassin associées aux plaques ont montré qu'elles constituaient des techniques fiables au minimum sur le moyen terme.

Patients who are suffering from a significant bone loss of the proximal femur, whether tumor­ derived or loosening of a primary implant,should benefit a reconstruction.This can be achieved by the use of a megaprosthesis or a composite prosthesis made with a massive proximal femoral allograft. At present, the scientific literature does not completely disentangle these two surgical approaches in terms of prosthetic survival. Les patients qui souffrent d'une perte de substance importante du fémur proximal,- qu'elle soit d'origine tumorale ou par déscellement d'un implant primaire, doivent bénéficier d'une reconstruction.Celle-ci peut être réalisée par l'utilisation d'une prothèse massive ou d'une prothèse composite combinée à une allogreffe. À l'heure actuelle, la littérature scientifique ne permet pas de départager radicalement ces deux approches chirurgicales en terme de survie prothétique.La technique de reconstruction combinée avec une allogreffe apporte cependant un iéger avantage en terme de restauration de la fonction ainsi que du capital osseux.Nous réalisons une étude rétrospective continue sur quarante-cinq patients qui ont bénéficié de ce type de reconstruction par allogreffe, afin d'évaluer les complications et les facteurs affectant la durabilité de la reconstruction.
