Abstract

The incidence of cancer diagnosed during pregnancy is relatively rare: about 1 out of 1000 pregnant women. However, it always represents a difficult situation because both the mother and the baby are concerned. When a cancer is diagnosed, several therapeutic strategies can be envisaged: surgery, radiotherapy or chemotherapy either during pregnancy or alter the child's birth. The first option requires a serious evaluation of the risks for the mother and the development of the fetus and of the child after birth. When chemotherapy is considered, risks are: neonatal death, congenital malformations, preterm births, low birth's weights and others problems during child's development. For the mother, there are some problems too. In particular, during the pregnancy, various parameters influencing pharmacokinetics are altered and drug administration becomes more difficult to handle. Generally, cytotoxic antitumor drugs are not given during the first trimester of the pregnancy because it's the period of organogenesis and the risk of malformations is therefore higher. Chemotherapy is often initiated during the second and third trimesters to reduce such risks for the child. Important questions are to know to which extent anticancer drugs are teratogens and whether some should be preferred to others. This requires the assessment of the transplacental transfer of drugs and the capacity to prevent or correct their side effects. Studies have been performed on animals, usually mice, rats or rabbits, and can reasonably be extrapolated to humans, but with a great caution.
