Abstract

Objectives: We aimed to compare the sélective feticide (SF) practice in the department of obstetrics of the Cliniques universitaires saint-Luc in Brussels with the literature data and Dr Amaral’s conclusions. The final goal was to update our SF treatment algorithm.Methods: It is a rétrospective study based on antenatal and neonatal data of 50 SF cases which occurred in our center between 1996 and 2011. The pregnancies where split in 2 groups following their chorionicity. Dichorionic pregnancies where managed by intracardiac KC1 injection. Monochorionic pregnancies where managed either by radiofrequency (RFA), bipolar cord coagulation (BCC), Alcohol or Histoacryl injection. The neonatal follow up was analyzed and compared with the literature variables and the Dr. Amaral’s conclusions.Results: Thirty KC1 injections where performed. We observed a mortality rate of 16%,55.56% of prematurity and 14,43% of PPROM. We could not put in evidence any disadvantageous period for KC1 injection. 20 Monochorionic pregnancies underwent SF: 12 BCC, 2 RFA, 4 Alcohol injections and 2 of Histoacryl. We observed 23,81% percent of mortality and PPROM and 64,71% of prematurity. Alcohol and Histoacryl injection were stopped because of the poor outcomes. The 2 cases of RFA had a good follow-up. BCC is the most common technique in our center with 15,38% of mortality. No disadvantageous gestational period for SF was detected on monochorionic pregnancy whatever the technique used.Discussion: Our results are similar to those of the literature. KC1 injection is the gold standard for SF on dichorionic pregnancy and the literature does not report any disadvantageous period for its performance. RFA and BCC are the 2 technique used for monochorionic SF. However, literature reports a lack of effectiveness for the BCC before 17 to 19 weeks of amenorrhea.Conclusion: Our new algorithm advises KC1 injection for ail dichorionic pregnancies without any limited period. Monochorionic pregnancies should be treated by RFA before 18 weeks and by BCC after. Objectifs : L’objectif de notre étude est de comparer la pratique de l’intervention sélective de grossesse (ISG) dans le service d’obstétrique des cliniques universitaires saint-Luc à Bruxelles à la littérature et aux conclusions du Dr. Amaral. Le but final est de faire évoluer l’algorithme de prise en charge des ISG réalisées dans notre centre. Méthodes : Il s’agit d’une étude rétrospective portant sur les données anténatales et néonatales de 50 cas d’ISG réalisées dans notre centre entre 1996 et 2011. Les grossesses ont été séparées selon leur chorionicité. Les grossesses bichoriales ont été opérées par injection de KCI. Les grossesses monochoriales l’ont été soit par radiofréquence (RFA), pince bipolaire (BCC), injection d’Alcool ou d’Histoacryl. Le devenir post opératoire a été analysé et comparé suivant les variables étudiées dans la littérature et par le Dr. Amaral. Résultats : Trente injections de KCI ont été réalisées. Le taux de réussite de l’intervention était de 100%. Nous avons obtenus un taux de mortalité de 16%, de prématurité de 55,56% et de RPPE de 14,43%. Nous n’avons pas pu détecter de période défavorable à l’injection de KC1 dans notre étude. 20 ISG ont été réalisées sur grossesse monochoriale : 12 BCC, 2 RFA, 4 injections d’Alcool et 2 d’Histoacryl. Nous avons obtenus un taux de mortalité et de RPPE de 23,81% et 64,71% de prématurité. L’injection d’Alcool et d’Histoacryl ont été arrêtées, suite aux mauvais résultats rapportés par la littérature. Les 2 cas de RFA ont eu un devenir post opératoire satisfaisant. La BCC est la technique la plus employée par notre centre et obtient un taux de mortalité de 15,38% Nous n’avons pas détecté de période défavorable à la pratique de F ISG sur grossesse monochoriale et ce qu’importe la technique. Discussion : Qu’importe le type de grossesse, notre centre se situe dans les moyennes de la littérature. L’injection de KCI reste la technique de référence pour les ISG sur grossesse bichoriale et aucune période défavorable n’est rapportée par la littérature. La RFA et la BCC sont les 2 techniques de référence pour les ISG sur grossesse monochoriale. La littérature rapporte une moins bonne efficacité de la BCC avant 17 à 19 semaines d’aménorrhées.Conclusion : Notre nouvel algorithme recommande l’injection de KCI pour les grossesses bichoriales et ce à tout âge gestationnel. Pour les grossesses monochoriales, la RFA est la technique de référence avant 18 semaines d’aménorrhées et la BCC passé cette date.
