Abstract

La maladie de Parkinson est caractérisée par une perte des neurones dopaminergiques, ce qui entraine une perte des transporteurs de la dopamine (DaT) dans le striatum. Son diagnostic est essentiellement clinique mais cependant il peut être difficile surtout au stade précoce de la maladie et chez les patients ne présentant pas les symptômes classiques. Récemment, le DaTSCAN, un radioligand du DaT a été mis sur le marché. Couplé à la tomographie d'émission monophotonique (TEMP), il permet de visualiser la quantité de DaT encore présente. Le but de ce travail était d'évaluer l'intérêt du DaTSCAN dans le diagnostic et le suivi de la maladie de Parkinson. Dans une étude réalisée chez des patients avec un diagnostic clinique incertain, en prenant comme référence le diagnostic clinique posé à 36 mois, l'imagerie au DaTSCAN a montré une sensibilité de 78% et une spécificité de 97% système dopaminergique, ce qui soutient le diagnostic de la maladie de Parkinson, du parkinsonisme atypique ou de la démence à corps de Lewy. Tandis qu'une image normale indique qu'il n'y a pas de neurodégénérescence et qu'il ne s'agit pas de la maladie de Parkinson mais bien des syndromes parkinsoniens tels que le tremblement essentiel ou le parkinsonisme iatrogène. Cependant, l'imagerie au DaTSCAN ne permet pas le diagnostic différentiel entre la maladie de Parkinson et les formes de parkinsonisme atypique. Enfin, en plus de ses applications en pratique clinique, le DaTSCAN pourrait s'avérer intéressant dans le cadre d'études évaluant les traitements neuroprotecteurs. Une étude réalisée chez 20 patients au stade précoce de la maladie de Parkinson a démontré que le DaTSCAN pouvait mesurer le taux de progression de la dégénérescence dopaminergique. La diminution de la captation du radiotraceur (et donc la perte neuronale) a été estimée à 8% par an. Parkinson's disease is characterized by degeneration of dopaminergic neurons, resulting in loss of dopamine transporters in the striatum. The diagnosis of Parkinson's disease is mainly clinical but it can be very challenging, especially in the early stage of the disease and in patients who have not classic symptoms. Recently, DaTSCAN, a radioligand of the DaT was licensed. Using single-photon emission computed tomography (SPECT), it allows to visualize the amount of DaT present. The aim of this work was to assess the benefit of DaTSCAN on the diagnosis and monitoring of Parkinson's disease. In patient with clinically uncertain parkinsonism , baseline imaging with DaTSCAN has shown 78% sensitivity and 97% specificity with reference to clinical diagnosis at 36 months, versus 93%and 46%, respectively, for baseline clinical diagnosis. Abnormal imaging indicates neurodegeneration of dopaminergic system, supportive of a diagnosis of Parkinson's disease, atypical parkinsonism or dementia with Lewy bodies. Normal imaging supports diagnosis of a condition not involving neurodegeneration such as essential tremor or drug-induced parkinsonism. However, DaTSCAN is not helpful in differentiating between Parkinson's disease and atypical parkinsonism. Finally, in addition to its application in clinical practice, DaTSCAN might be interesting in the context of studies evaluating the neuroprotective treatments. A study of 20 patients with early Parkinson's disease showed that DaTSCAN could measure the rate of dopaminergic degeneration's progression. The annual decrease in striatal DaTSCAN binding was about 8%.

Abstract

This new project on PET/MR imaging in oncology includes digital interactive software matching the cases in the book. The interactive version of the atlas is based on the latest web standard, HTML5, ensuring compatibility with any computer operating system as well as a dedicated version for Apple iPad and iPhone. The book opens with an introduction to the principles of hybrid imaging that pays particular attention to PET/MR imaging and standard PET/MR acquisition protocols. A wide range of illustrated clinical case reports are then presented. Each case study includes a short clinical history, findings, and teaching points, followed by illustrations, legends, and comments. The multimedia version of the book includes dynamic movies that allow the reader to browse through series of rotating 3D images (MIP or volume rendered), display blending between PET and MR, and dynamic visualization of 3D image volumes. Abstract
