Abstract

La respirazione meccanica è una procedura con cui ogni medico che si occupi di medicina intensiva, di discipline chirurgiche e di medicina generale deve quotidianamente confrontarsi. Questo volume si propone di fornire ai lettori le più aggiornate conoscenze necessarie all’applicazione di questa procedura, insieme a una valutazione critica dei relativi vantaggi e svantaggi basata sull’evidenza. La prima versione in lingua italiana del volume, pubblicata nel 2007, ha ottenuto un grande successo ed è diventata un importante riferimento per tutti i professionisti che si trovano a utilizzare questa pratica clinica. Per questo motivo è stato deciso di realizzare questa seconda edizione, nella quale i più recenti progressi in ambito clinico e terapeutico, così come l’eliminazione di concetti oggi superati, vanno ad aggiornare la struttura originaria: partendo dalle basi anatomiche e fisiologiche della respirazione meccanica e dalle varie forme di insufficienza respiratoria, vengono prese in considerazione le indicazioni a questo tipo di intervento, le complicazioni ad esso legate, oltre che i metodi alternativi e i suggerimenti sulla gestione delle malattie specifiche dell’apparato respiratorio. Le più recenti tecniche di terapia respiratoria e di respirazione meccanica non invasiva completano questo trattato.

Abstract

Inadequate humidification of inspired gases can cause a variety of serious problems, and humidification has accordingly become an important aspect of modern intensive care medicine. The array of available devices is now formidable, and appropriate advice on their use is essential. Accordingly, this book is designed to serve as a practical guide for clinicians, providing information on the theoretical background of humidification, the equipment, and its optimal application. The book starts by examining the physiological basis of humidification. Current devices are then discussed, with careful attention to factors influencing their performance and methods to evaluate their effectiveness. The two scenarios of mechanical and non-mechanical ventilation are considered, and the issue of ventilator-associated pneumonia is addressed in detail. Further chapters focus on such topics as humidification following tracheostomy, humidification of the artificial airway during secretion management, measurement of inspired gas temperature in the ventilated neonate, and humidification in the home care setting.
