Tant l’évolution médicale que les développements technologiques et thérapeutiques orientent au fil des années les pratiques médicales.
C’est dans ce contexte de perpétuel progrès qu’un double constat a été établi au sein du département pédiatrique du CHU St-Pierre où j’ai exercé ma profession jusqu’au 15 août 2002 et a suscité la réalisation de ce travail.
Tout d’abord, on enregistre au cours de ces dernières années une croissance continue de la fréquentation des services d’urgence. Par ailleurs, à l’heure actuelle, de nombreux schémas thérapeutiques validés permettent dans certains cas une prise en charge prolongée au sein même du service des urgences. Ces deux phénomènes concourent donc à une surpopulation de ce type d’unités qui n’est pas sans conséquences au niveau de l’organisation et de la qualité des soins.
De plus, en ce qui concerne plus particulièrement l’unité d’hospitalisation de pédiatrie, on note un nombre non négligeable de séjours de très courte durée (24 à 48 H) qui engendrent un turn over important et donc des fréquentes difficultés d’organisation. Cette réduction est notamment liée à l’évolution des protocoles de prise en charge de certaines pathologies comme par exemple les modalités de réhydratation orale dans le cadre des gastro-entérites aigües.
Face à cet état des choses, il nous a semblé opportun d’entamer une réflexion afin d’adapter les structures et le fonctionnement hospitalier à ces changements.
Parmi les diverses initiatives actuellement en développement dans l’organisation hospitalière, le concept d’hospitalisation provisoire nous est apparu comme une alternative au potentiel encore peu exploité dans le secteur pédiatrique de notre pays. Elle offre pourtant la possibilité de mener à bien, au sein d’une structure médicalisée en continu, les missions de diagnostic et d’évaluation, dans un laps de temps réduit, des situations de gravité modérée, et la mise en place d’une thérapeutique efficace afin soit d’autoriser le retour à domicile au terme d’un délai ne dépassant pas quelques heures soit d’orienter correctement le patient vers le service le plus adapté à son état cliniques.
Au cours de ce travail, nous allons tenter de concrétiser ce concept très séduisant.
Nos objectifs s’articulent sur deux axes principaux :
- Premièrement, la pertinence de la création d’une telle structure. Est-ce vraiment un volet de la « solution » à la problématique soulevée ?
- Deuxièmement, la faisabilité de cette unité en termes d’organisation, de financement, d’agrément.
C’est tant par une approche théorique que pratique que nous tenterons d’éclairer ces différents aspects

PVM (Parallel Virtual Machine) and MPI (Message Passing Interface) are the most frequently used tools for programming message passing applications. This volume includes the selected contributions presented at the 10th - ropean PVM/MPI Users’ Group Meeting (Euro PVM/MPI 2003), which was held in Venice, Italy, September 29–October 2, 2003. The conference was jointly organized by the Department of Computer Science of the Ca’ Foscari University of Venice, Italy and the Information Science and Technologies Institute of the National Research Council (ISTI-CNR), Pisa, Italy. TheconferencewaspreviouslyheldinLinz,Austria(2002),Santorini,Greece (2001), Balatonfured, ¨ Hungary (2000), Barcelona, Spain (1999), Liverpool, UK (1998), and Krakow, Poland (1997). The ?rst three conferences were devoted to PVM and were held in Munich, Germany (1996), Lyon, France (1995), and Rome, Italy (1994). The conference has become a forum for users and developers of PVM, MPI, and other message passing environments. Interactions between these groups has proved to be very useful for developing new ideas in parallel computing, and for applying some of those already existent to new practical ?elds. The main topics of the meeting were evaluation and performance of PVM and MPI, ext- sions, implementations and improvements of PVM and MPI, parallel algorithms using the message passing paradigm, and parallel applications in science and engineering. In addition, the topics of the conference were extended to include Grid computing, in order to re?ect the importance of this area for the hi- performance computing community.
