Abstract

Are new reproductive and genetic technologies racing ahead of a society that is unable to establish limits to their use? Have the "new genetics" outpaced our ability to control their future applications? This book examines the case of preimplantation genetic diagnosis (PGD), the procedure used to prevent serious genetic disease by embryo selection, and the so-called "designer baby" method. Using detailed empirical evidence, the authors show that far from being a runaway technology, the regulation of PGD over the past fifteen years provides an example of precaution and restraint, as well as continual adaptation to changing social circumstances. Through interviews, media and policy analysis, and participant observation at two PGD centers in the United Kingdom, Born and Made provides an in-depth sociological examination of the competing moral obligations that define the experience of PGD. Among the many novel findings of this pathbreaking ethnography of reproductive biomedicine is the prominence of uncertainty and ambivalence among PGD patients and professionals--a finding characteristic of the emerging "biosociety," in which scientific progress is inherently paradoxical and contradictory. In contrast to much of the speculative futurology that defines this field, Born and Made provides a timely and revealing case study of the on-the-ground decision-making that shapes technological assistance to human heredity.

Abstract

Proposals for screening pregnant women and newborns for HIV infection have provoked much controversy. Abstract

Advances in reproductive and perinatal medicine have given rise to difficult ethical issues. Do all women have the right to choose whether to reproduce? What is the moral status of the foetus during various stages of gestation and what obligations do parents have to the foetus during this period? In this book Carson Strong develops an ethical framework that aims to help resolve these and many other issues of concern to health professionals, policymakers and the general public. Strong begins by exploring the significance of reproductive freedom, drawing on constitutional law and feminist writings, among other sources. Next he assesses the moral status of offspring during pre-embryonic, embryonic, foetal and postnatal stages of development, discussing the obligations of procreators during gestation, and offering a perspective on the idea that the moral standing of the foetus increases as foetal development proceeds. He then suggests an approach to the question of how priorities should be assigned to conflicting values, one that draws on a version of casuistic reasoning.In the second half of the book, Strong applies this ethical framework to some of the problematic areas in reproductive and perinatal medicine. These include prenatal genetic testing for susceptibilities to common diseases and for "enhancement" of offspring; research using pre-embryos and embryos; non-traditional family arrangements, such as surrogate motherhood and ovum donation for older women; and treatment of foetuses with anomalies. His discussion takes into account the clinical dimensions of issues and reflects a consideration of the ethical, legal, medical and psychosocial literature. Drawing on the law and feminist writings, this text explores the significance of reproductive freedom. It assesses the moral status of offspring in pre-embryonic, embryonic, foetal and postnatal development and applies an ethical framework to problematic areas in reproductive medicine.

Abstract

A study of the importance of self-trust for women's autonomy in reproductive health. The power of new medical technologies, the cultural authority of physicians, and the gendered power dynamics of many patient-physician relationships can all inhibit women's reproductive freedom. Often these factors interfere with women's ability to trust themselves to choose and act in ways that are consistent with their own goals and values. In this book Carolyn McLeod introduces to the reproductive ethics literature the idea that in reproductive health care women's self-trust can be undermined in ways that threaten their autonomy. Understanding the importance of self-trust for autonomy, McLeod argues, is crucial to understanding the limits on women's reproductive freedom. McLeod brings feminist insights in philosophical moral psychology to reproductive ethics, and to health-care ethics more broadly. She identifies the social environments in which self-trust is formed and encouraged. She also shows how women's experiences of reproductive health care can enrich our understanding of self-trust and autonomy as philosophical concepts. The book's theoretical components are grounded in women's concrete experiences. The cases discussed, which involve miscarriage, infertility treatment, and prenatal diagnosis, show that what many women feel toward themselves in reproductive contexts is analogous to what we feel toward others when we trust or distrust them. McLeod also discusses what health-care providers can do to minimize the barriers to women's self-trust in reproductive health care, and why they have a duty to do so as part of their larger duty to respect patient autonomy.
