Abstract

This open-access book analyses how stakeholder relationships impact the sustainability of health aid. It does this by providing an overarching analytical framework, which allows for a systematic analysis of sustainability, relationships, and a possible causal link between these phenomena. The book goes beyond universal paradigms and detailed single-case studies by offering a thorough analysis of development projects to identify the factors that are also applicable to similar initiatives in comparable contexts. Empirically, it focuses on two health initiatives, both implemented in the Kyrgyz Republic, a country pursuing a sector-wide approach to health aid. Unique primary material provides insights into a geographic region that is mostly neglected, and will be of interest to students and researchers of social policy, development studies, international health and those focusing on the post-Soviet region and Central Asia.

Abstract

The study assesses seven cases of nation-building operations following major conflicts: Germany and Japan immediately after World War II; Somalia, Haiti, and Kosovo in the 1990's; and Afghanistan and Iraq since 2001. It concludes that two factors increase the likelihood of successful health outcomes: planning and coordination, and infrastructure and resources. In addition, the study argues that health can have an independent impact on broader political, economic, and security objectives during nation-building operations.

Abstract

Employing a Zellner-type indirect regression technique, and data from the 1972 California Copayment Experiment, the authors attempt to assess the impact of a copayment requirement on utilization of health care resources by the poor. Focus is on three questions regarding effects of an increase in out-of-pocket cost of physician office visits: (1) Will such an increase inhibit demand for ambulatory care? (2) Will it increase or decrease demand for hospitalization? (3) How will it affect total resource cost of health care services, both in and out of hospitals? The results indicate that a $1 copayment requirement apparently decreases demand for physician visits by 8 percent and increases demand for hospital inpatient services by 17 percent. Although the confidence intervals are large, point estimates indicate that copayment increases overall program costs by a statistically insignificant 3 to 8 percent. Thus copayments could be self-defeating as a method of controlling medical costs in a welfare population.

Au début du XXe siècle, les politiques de santé évoluent au gré des bouleversements des sociétés civiles confrontées au choc des armées européennes. En Grèce, les Guerres Balkaniques, la Première Guerre mondiale puis l’expédition d’Asie Mineure imposent dix années de conflit (1912-1922) durant lesquelles les mouvements de troupes et de réfugiés comme la détresse des populations forment le terreau d’épidémies dévastatrices. À cette situation de crise sanitaire, sociale et démographique, répondent pour la première fois des politiques volontaristes de prise en charge médicale et de contrôle social, dans le sillage des théories hygiénistes de la fin du XIXe siècle. Si leurs réalisations restent limitées durant la période, elles posent les bases administratives, organisationnelles et idéologiques du système de soins moderne. Cet ouvrage propose une étude de ce processus à travers l’influence de la mission militaire française puis de l’Armée française d’Orient, mettant en lumière les interactions complexes des acteurs grecs et étrangers impliqués dans cette « guerre totale ». At the start of the 20th century, the upheaval in civil societies that were confronted with the shock of European armies led to a change in public health policy. In Greece, the Balkan wars, the First World War and the Greco-Turkish War brought ten years of conflict (1912–1922) during which movements of troops and refugees and a generally distressed population provided fertile ground for devastating epidemics. For the first time ever, this health, social and demographic crisis led to the advent of pro-active medical and social policies, based on late 19th century research into public health.
