Abstract

Alcohol is the sixth leading risk factor for disability and premature death all over the world, and one of the leading causes of premature mortality in western societies; it is a leading risk factor for death in young and middle-age males. Heavy drinking accounts for about two thirds of the burden of disease attributable to alcohol. In the early 1980s, screening and brief interventions (SBI) in primary health care settings were proposed as effective strategies to identify risky drinkers and to help them reduce their drinking. Since then, a growing body of evidence, including several meta-analysis and Cochrane reviews, has shown the efficacy and effectiveness of SBI in primary health settings. However, demonstrating the effectiveness of SBI has not been insufficient to facilitate its general implementation in the routines of primary health care physicians, and in fact the dissemination of SBI has proven to be a difficult business. Qualitative and quantitative research has identified most of the facilitators and barriers for its implementation, and publicly funded research has been earmarked to address the dissemination problems worldwide. Some examples are the World Health Organization Phase III and Phase IV studies on the identification and management of alcohol-related problems in primary care, EU funded projects (PHEPA, AMPHORA, ODHIN, BISTAIRS), the UK SIPS trials and the SBIRT developments sponsored by the Substance Abuse & Mental Health Services Administration (SAMHSA) in the USA. The efficacy and effectiveness of SBI in primary health is now well established, but there are still some questions that remain unsolved: which practitioners should deliver them; what length should they be; is there a need for booster sessions; is there added value of a motivational approach? These questions, together with other relevant aspects of SBI, need ongoing research. In recent years, SBIs have been tested in settings other than primary health care, including hospitals, accident and emergency rooms, criminal justice, colleges and universities, social services and pharmacies. In some of those areas, the evidence is scarce (for example, pharmacies) while in others it is very promising (for example, students and hospitals). New technologies have also offered the possibility of online tools, and, in the last few years, different digital-based applications have been tested successfully as new ways to deliver effective SBIs to larger amounts of people. Brief interventions have also spread to drugs other than alcohol. This book aims to be an update of the state-of-the art of brief advice. It is a compilation of articles published by some of the most relevant researchers in the field in Frontiers in Psychiatry between 2014 and 2016.

Abstract

Alcoholic beverages, and their harmful use, have been familiar fixtures in human societies since the beginning of recorded history. Worldwide, alcohol is a leading cause of ill health and premature mortality. It accounts for 1 in 17 deaths, and for a significant proportion of disabilities, especially in men. In OECD countries, alcohol consumption is about twice the world average. Its social costs are estimated in excess of 1% of GDP in high- and middle-income countries. When it is not the result of addiction, alcohol use is an individual choice, driven by social norms, with strong cultural connotations. This is reflected in unique patterns of social disparity in drinking, showing the well-to-do in some cases more prone to hazardous use of alcohol, and a polarisation of problem-drinking at the two ends of the social spectrum. Certain patterns of drinking have social impacts, which provide a strong economic rationale for governments to influence the use of alcohol through policies aimed at curbing harms, including those occurring to people other than drinkers. Some policy approaches are more effective and efficient than others, depending on their ability to trigger changes in social norms, and on how well they can target the groups that are most at risk. This book provides a detailed examination of trends and social disparities in alcohol consumption. It offers a wide-ranging assessment of the health, social and economic impacts of key policy options for tackling alcohol-related harms in three OECD countries (Canada, the Czech Republic and Germany), extracting relevant policy messages for a broader set of countries.

Abstract

In Teaching 'Proper' Drinking?, the author brings together three fields of scholarship: socio-historical studies of alcohol, Australian Indigenous policy history and social enterprise studies. The case studies in the book offer the first detailed surveys of efforts to teach responsible drinking practices to Aboriginal people by installing canteens in remote communities, and of the purchase of public hotels by Indigenous groups in attempts both to control sales of alcohol and to create social enterprises by redistributing profits for the community good. Ethnographies of the hotels are examined through the analytical lens of the Swedish 'Gothenburg' system of municipal hotel ownership. The research reveals that the community governance of such social enterprises is not purely a matter of good administration or compliance with the relevant liquor legislation. Their administration is imbued with the additional challenges posed by political contestation, both within and beyond the communities concerned.

Abstract

The Global status report on alcohol and health (2011) presents a comprehensive perspective on the global, regional and country consumption of alcohol, patterns of drinking, health consequences and policy responses in Member States. It represents a continuing effort by the World Health Organization (WHO) to support Member States in collecting information in order to assist them in their efforts to reduce the harmful use of alcohol, and its health and social consequences. The report was launched in Geneva on Friday 11 Februray 2011 during the first meeting of the WHO global counterparts for implementation of the global strategy to reduce the harmful use of alcohol.

Abstract

Diet, physical activity, and body weight status (including body composition) are increasingly recognized as key factors that influence cancer across its continuum. Observational studies as well as basic research in cell culture and animal models provide evidence that several mononutrients and phytochemicals play a protective role either in hindering normal cells from transforming to precancerous lesions or in slowing the dysregulated cell growth that occurs in the later stages of disease. Similar evidence exists for physical activity and body habitus. As findings from these studies emerge, interventions are designed to ultimately test the impact of various dietary and exercise regimens directly on populations at risk – whether that be in individuals who are cancer-free but who may have increased risk due to family history, or in cancer survivors who are at risk for cancer progression or the occurrence of a new second malignancy. This book includes manuscripts that focus on diet, physical activity, and/or weight status in relation to cancer prevention and control, as well as symptom management. The order of articles follows the cancer continuum. The book begins with the role of diet and exercise in the primary prevention of cancer in both normal and high-risk individuals, and then focuses on preventing neoplastic progression in those who are newly diagnosed with the disease. Later chapters center on dietary and physical activity as key factors in cancer survivorship, and finally concluding with works attributing dietary and physical activity factors on cancer survival.

Abstract

Alcohol is often perceived as an under-rated risk factor for human health. This book corrects these misperceptions and misinformation by providing up to date reviews and publications that consider the impact of alcoholic beverages on human health in the domains of toxicity, carcinogenicity, genotoxicity, foetal toxicity, neurotoxicity, impacts of alcohol on the gastro-intestinal system (including nutrient deficiencies), cardiovascular system, injuries, body weight and communicable diseases. The reprint considers how the impact of alcohol on human health can be mitigated – through, for example, improved labelling on nutrients and health warnings, better policy measures, and actions by alcohol producers on their products through reformulation to lower alcoholic strength.
