Voor mensen met een acuut hartinfarct is een vroegtijdige diagnose door middel van een elektrocardiogram (ecg) en een coronair angiogram van levensbelang. Door een vroege diagnose kan snel worden besloten om acute percutane coronaire interventie (PCI) toe te passen. Een PCI-behandeling maakt het bloedvat weer snel doorgankelijk, waardoor schade aan het hart wordt beperkt. Onderzoek heeft aangetoond dat een acute PCI de overlevingskans en de kwaliteit van leven na het infarct aanmerkelijk vergroot. Het ecg en het angiogram bij een acuut hartinfarct geeft de hulpverlener handvatten om snel een goede diagnose te stellen en therapie te kunnen starten. Het boek geeft inzicht in de elektrocardiografische patronen van het acute hartinfarct. Hierbij is gebruikgemaakt van de gegevens van negentien patiënten die de afgelopen jaren in het Universitair Medisch Centrum Groningen zijn behandeld met een acute PCI. Per hoofdstuk wordt het ziektebeeld van één patiënt besproken. Aan de orde komen de klinische presentatie, het ecg vóór de behandeling, de kerngegevens van de behandeling, zoals vooral zichtbaar op het coronaire angiogram vóór en na de PCI, de effecten van de PCI, zoals zichtbaar op het ecg na de behandeling, en het verdere klinische beloop. Het boek geeft hiermee een breed en divers beeld van de dagelijkse praktijk. Dit boek is bedoeld voor de vele hulpverleners die betrokken zijn bij de prehospitale fase, de acute behandeling en de herstelperiode van een acuut hartinfarct, met name huisartsen, ambulancepersoneel, SEH-artsen, coassistenten, internisten (in opleiding) en cardiologen (in opleiding). The mortality from ischemic heart disease has decreased in recent years. The better understanding of risk factors associated with development of coronary artery disease has significantly contributed to this decline. Improvements in medical and interventional therapy have reduced the complications associated with acute myocardial infarction as well as revascularization. After the introduction of imaging modalities, the noninvasive characterization of regional function, perfusion and metabolism allowed for more sophisticated tissue characterization to identify reversible dysfunction with high diagnostic and prognostic accuracy. We now can legitimately claim that computed tomography angiography (CTA) of the coronary arteries is available. In the evaluation of patients with suspected coronary artery disease, many guidelines today consider CTA an alternative to stress testing. However the nuclear technique most frequently used by cardiologists is myocardial perfusion imaging (MPI). The combination of a nuclear camera with CTA allows for the attainment of coronary anatomic, cardiac function and MPI from one piece of equipment. Assessing cardiac viability is now fairly routine with these enhancements to cardiac imaging. Traditional coronary angiography presents a variety of limitations related to image acquisition, content, interpretation, and patient safety. Barriers to such improvements include the paucity of clinical outcomes studies related to new imaging technology, the need for physician and staff member training, and the costs associated with acquiring and effectively using these advances in coronary angiography. This issue is full of important information that every cardiologist needs to now.

Atherosclerotic cardiovascular disease is still the most common cause of death among adults. Its prevalence is increasing in developing countries and despite all advances in both diagnostic tools and treatment modalities, it is still very common in the developed world. Obesity, diabetes mellitus, hypercholesterolemia and overuse of dietary salt play a pivotal role in increased cardiovascular morbidity and mortality worldwide. Current clinical efforts are mainly focused on the diagnosis and treatment of myocardial infarction. In this book, we provide epidemiological data on myocardial infarction and atherosclerotic cardiovascular disease, current diagnostic biochemical tests and management strategies. A specific patient group, children, experiencing myocardial infarction are also addressed. Current advances in the management of myocardial infarction have decreased the morbidity and mortality from atherosclerotic cardiovascular disease and especially myocardial infarction; however, more can be achieved by the prevention of atherosclerotic processes via focusing on the early stages of the disease.
