This book presents more than 120 cases covering all areas of reconstructive surgery. Divided into six parts (Head/Neck, Upper Extremity, Lower Extremity, Trunk, Breast and Lymphedema), it guides the reader through the difficult path of problem diagnosis, analysis and decision-making, presenting concrete steps and techniques for the successful management of patients with specific reconstructive needs. Each full-color case starts with a patient profile and continues with the diagnosis, key decisions, treatment plan, surgical procedure(s) and technical steps, postoperative management, outcome and case conclusion. Further, each case includes a discussion of pros and cons, comments, learning points and suggestions for further reading. This book will be useful for all surgeons actively involved or interested in Reconstructive Microsurgery, especially Senior Residents and Fellows in Plastic, Head and Neck, Breast Surgery and Orthopedics.

Cubital tunnel syndrome is the second most common compression neuropathy in the upper extremity. While the treatment of carpal tunnel syndrome is relatively straightforward, there is much debate regarding the most efficient diagnostic methods, appropriate non-surgical management, and surgical management of cubital tunnel syndrome. This unique book is sensibly divided into three thematic sections. Part one reviews the relevant anatomy and presents the physical exam and diagnostic test modalities, along with non-surgical treatment strategies such as splinting and injections as well as the role of physical therapy. Surgical treatment strategies are discussed in detail in part two, including decompression, anterior transposition, minimal medial epicondyectomy and ulnar motor nerve transfer. Management of the failed release is highlighted here as well. Part three describes outcomes, acute and chronic complications and rehabilitation. Case material will be included where appropriate to provide real-world illustration of the presentations and procedures discussed. Practical yet comprehensive, Cubital Tunnel Syndrome will be an excellent resource for orthopedic, hand and plastic surgeons, trainees and residents, with content that will also be useful for physical therapists and rehabilitation specialists.

Presenting step-by-step procedures written by experts in the field, this comprehensive clinical guide discusses the diagnosis (electrodiagnostic and ultrasound) and management of compressive neuropathies of the upper extremity. Compressive (or compression) neuropathy, also known as entrapment neuropathy or trapped nerve, is a common condition of the upper extremity in which the nerves of the arm – median, ulnar and radial being the most common – are compressed, causing pain and discomfort as well as possible pathological and anatomical changes. Carpal and cubital tunnel syndrome are the most well-known and treated, with nerve release and decompression surgeries being the usual treatment, though the variety of neuropathies and management strategies goes beyond these conditions. Chapters included describe in detail the latest, cutting-edge management strategies for the various manifestations of compressive neuropathy of the hand and wrist – carpal tunnel syndrome, cubital tunnel syndrome, ulnar nerve syndrome, radial tunnel syndrome, pronator teres syndrome, Wartenberg’s syndrome, thoracic outlet syndrome and suprascapular neuropathy – as well as revision carpal and cubital tunnel surgical treatment options. Plentiful intraoperative photos and detailed illustrations, along with clinical case material and pearls and pitfalls, make this the ideal resource for orthopedic, hand and plastic surgeons aiming for the most optimal outcomes.

Arthroplasty of the upper extremity is an established surgical intervention in the management of arthritis of the elbow, wrist and hand. The anatomy, kinematics and demands of the elbow, wrist, thumb CMC, and finger MCP and PIP joints pose unique surgical challenges. Implant design considerations are important in providing a joint that mimics the native joints and maximizes survivorship. However, outcomes are less predictable in these upper extremity joints when compared to the hips and knees. Each joint also carries its own set of potential complications and salvage options for revision and failed arthroplasty. This unique text helps the orthopedic and hand surgeon understand the surgical approaches, unique anatomic considerations, and both the historical and current designs related to each respective joint, enabling the surgeon to better appreciate the benefits and limitations of each arthroplasty. Presenting the current state of the art, the seven sections proceed anatomically from the elbow to the fingers, with each section comprised of three thematic chapters discussing implant design considerations, primary arthroscopy techniques and revision arthroscopy techniques, including non-surgical options for treating these often difficult problems. This consistent approach, accompanied by plentiful figures, radiographs and intraoperative photos, ensures that this will be a user-friendly resource for orthopedic and hand surgeons, residents and trainees.

This book serves as an anatomic atlas of the nerves that innervate the joints of the human body in a format that also provides technical insight into pathways that both interventional pain management and surgical subspecialists can use to denervate those painful joints when traditional approaches to manage the pain are no longer successful. This book avails the knowledge of how denervation can relieve joint pain available to the many groups of physicians who care for this problem. Each chapter is devoted to a joint and reviews the neural anatomy as it relates to the clinical examination of the patient. Chapters are user friendly and provide details on the indicated nerve blocks and the clinical results of partial joint denervation. Clinical case studies also serve as a helpful guide in each chapter. Extensive intra-operative clinical photographs and photographs from new prosections provide examples to guide those physicians providing care to the patients with joint pain. Joint Denervation: Anatomic Atlas of Surgical Technique should be of interest to surgical subspecialists from Neurosurgery, Plastic Surgery, Hand Surgery, Orthopedic Surgery, Podiatric Foot & Ankle Surgery, and Oral & Maxillofacial Surgeons. It may also interest those physicians trained in Anesthesia, Radiology, and Physical & Rehabilitation Medicine for their evaluation and treatment protocols using hydrodissection, cryoablation and pulsed radiofrequency approaches to pain.

This book comprehensively covers hand trauma from initial presentation to management. Material is presented in an easy to follow practically applicable way, bridging the gap between what trainees encounter during training and what is encountered in a busy emergency department . Topics covered include upper limb anatomy, soft tissue injuries and infections, hand fractures, anaesthesia, and referrals from primary care, with detailed guides provided on how to successfully perform techniques such as digit replantation, extravasation, and locoregional flaps. Hand Trauma in Clinical Practice systematically describes a variety of categories of hand trauma, and plastic and orthopaedic surgical techniques. Therefore, it represents a valuable resource for practising and trainee plastic surgeons, orthopaedic surgeons, and emergency care specialists.

Comprised exclusively of clinical cases covering scaphoid fractures and nonunions, this concise, practical casebook will provide clinicians in orthopedic, plastic and hand surgery with the best real-world strategies to properly diagnose and treat the various forms of the condition they may encounter. After a brief introduction covering scaphoid anatomy, each chapter is in the form of a case that opens with a unique clinical presentation, followed by a description of the diagnosis, assessment and management techniques used to treat it, as well as the case outcome and clinical pearls and pitfalls. Cases included illustrate different injuries and management strategies from acute scaphoid fractures and nonunions to advanced collapse, covering non-operative and arthroscopic techniques as well as autograft procedures and salvage options. Pragmatic and reader-friendly, Scaphoid Fractures and Nonunions: A Clinical Casebook will be an excellent resource for seasoned as well as those training to be orthopedic surgeons, plastic surgeons, hand surgeons and emergency medicine physicians alike.
