Abstract

Dans la pratique du médecin stomatologiste ou du chirurgien dentiste, le geste chirurgical est indéniablement l'acte dominant. Néanmoins, une bonne connaissance des possibilités très variées offertes par la thérapeutique médicale devrait permettre de mieux répondre aux attentes des patients en complétant l'efficacité du geste chirurgical, voire en l'évitant dans certains cas. En particulier, l'importance de la pathologie de la muqueuse buccale, ses fréquentes difficultés de diagnostic et de traitement, son vécu souvent pénible pour le patient et son entourage rendent indispensable la prise en considération de l'ensemble des moyens thérapeutiques disponibles, des plus modestes aux plus élaborés. S'il existe de nombreux ouvrages de thérapeutique chirurgicale ou de pharmacologie et de remarquables traités de pathologie médicale ou chirurgicale dans le domaine buccodentaire, l'aspect pratique du traitement médical n'y est guère développé. Cet ouvrage rassemble, pour en faciliter l'usage, des notions souvent éparses dans la littérature scientifique, médicale (stomatologie, dermatologie, infectiologie, ORL, etc.), dentaire ou pharmaceutique. Les points suivants sont envisagés : thérapeutiques par voie locale (bains de bouche, collutoires, crèmes, solutions...), thérapeutique par voie générale (antibiotiques, anti-inflammatoires, antalgiques, sialomodulateurs ... ) ; hygiène, alimentation, thermalisme, kinésithérapie dentomaxillo-faciale ; utilisation de certains agents physiques ; principaux examens complémentaires. En annexe sont indiqués quelques prescriptions types, les interactions médicamenteuses à connaître, les principaux effets indésirables et contre-indications avec un rappel des règles de prescription ainsi que les effets des médicaments prescrits sur les contrôles anti-dopages.

Abstract

Fractures are a common finding in children and it is estimated that 2.1% of all children will suffer at least one fracture before the age of 16. With young children in particular, the question may arise if this is related to child abuse. The aim of this book is to help physicians involved in child abuse cases to interpret radiological findings in light of the forensic circumstances under which they occurred. The authors present up-to-date literature related to the mechanisms underlying non-accidental cases of trauma. In this book not only the radiological findings in child-abuse are discussed, but more importantly, these findings are analyzed from a forensic perspective. Careful attention is paid to evidence regarding reported trauma mechanisms and their clinical outcome; for example, can a fall from a couch result in a femoral fracture, and if not, where is the supporting evidence?
