Abstract

The history of mankind has been shaped by infections, more than by war and famine together. At the same time, however, the development of society has had an equally important effect on human diseases. The emergence of agriculture, urban societies and high population densities has been proven to be crucial for the spread of pathogens, and thus human action is currently the single most important driver of infectious epidemiology. Even today, where once major killers such as poliomyelitis have been eradicated, new pathogens are appearing as result of human activity. One such group of pathogens are the fungi, whose emergence is mainly due to modern medical practices. Fungal microorganisms, from yeasts colonizing the skin or mucosa, to molds from soil or water, are usually harmless in the context of normal host responses. However, the success of chemotherapy, as well as the AIDS pandemia, has led to immune deficiencies in a significant segment of the patient population, and the extensive use of intravenous catheters has provided a way of access for microorganisms which otherwise would find difficult to infect the host. As a result, a yeast such as Candida is now on the 4th place on the list of the most frequent sepsis agents, whereas infection with the mold Aspergillus is increasing in incidence and it is one of the most feared complications in patients with hematological malignancies.

Abstract

Fungal immunology, an expanding field of research, studies the interaction between fungi and the immune system. Modulated immunity as well as evasive mechanisms played by fungi all contribute to the chronicity and reoccurrence of fungal infections. Hypersensitivity to various fungal elements plays a major role in the pathogenesis of many diseases. This book will serve as a comprehensive review of all known immune mechanisms for common medically important fungal pathogens. Its novelty lies in the organization: Rather than chapters devoted to specific fungi, chapters are organized by organ system. All other texts in the field, which are now quite old, are arranged according to specific fungi. Organization by body system rather than fungi is important because one particular fungus can have a different effect on defense mechanisms in different parts of the body.

Abstract

Diagnosis and Treatment of Human Mycoses brings together globally-recognized mycoses experts to guide readers in the use of current knowledge in the field of medical mycology to manage those who suffer from fungal infections (mycoses). Often, diagnostic strategies and tests, including basic and directed culturing techniques, histopathology with standard and special stains, serological methods, and radiological studies all need to be considered and commonly combined to make the diagnosis of fungal infection. This volume first introduces and reviews these tools separately and then as they pertain to specific infections or groups of diseases. The volume consists of four parts. Parts I-III provide an overview of diagnostic and therapeutic tools, and part IV presents the human mycoses. Diagnosis and Treatment of Human Mycoses is meant to be a concise text that will provide the busy infectious disease, hematology–oncology, pulmonology, or critical care specialist a practical tool to diagnose and manage fungal infections.
