Generalized anxiety disorder (GAD) and panic disorder (PD) are two common mental illnesses that present in primary care clinics, often with physical symptoms that can inhibit appropriate diagnosis and treatment. Recognition of these disorders by primary care physicians is much lower than the expected rates--in part due to somatic presentations but also due to the lack of routine screening that is in place for some other mental illnesses. Patients with anxiety disorders are often high utilizers of health care resources, and when their anxiety disorders are not diagnosed and treated, they can frequently undergo more expensive testing to rule out medical causes. Identification of accurate and feasible screening instruments for GAD and PD that have been validated in primary care settings have the potential to improve detection and facilitate treatment of these disorders within the primary care clinic, or to generate appropriate referral. Our report is a systematic review of the literture to evaluate the performance of self-report instruments used to diagnose GAD and PD in primary care settings.

DSM-5 Manuel diagnostique et statistique des troubles mentaux, 5 ème édition qui sont chacun décrits de façon claire, concise et systématique (caractéristiques diagnostiques, prévalence, développement et évolution, facteurs de risque et pronostiques, questions diagnostiques liées à la culture ou au genre, retentissement fonctionnel, diagnostic différentiel, comorbidité). Le DSM-5 permet d'enregistrer les diagnostics avec les codes actuels de la classification internationale des maladies dde l'OMS (CIM-10). De plus, l'organisation des chapitres du DSM-5 a été conçue pour être compatible, autant que possible, avec la future CIM-11. Les troubles apparus dans l'enfance sont pris en compte dans une perspective développementale et logitudinale, ce qui aboutit à un chapitre sur les troubles neurodéveloppementaux et à la définition d'un "trouble du spectre de l'autisme". D'une façon plus générale le DSM-5 adopte une perspective développementale dans l'ordre de présentation des troubles. -Cetains chapitres sont nouveaux ou profondément remaniés: ex: les troubles de l'humeur du DSM-IV sont scindés en troubles bipolaires et troubles dépressifs, les troubles obsessionnels compulsifs et les troubles liés à des traumatismes ou à des facteurs de stress sont distingués des troubles anxieux, les troubles neurocognitifs intègrent des paramètres issus des recherches biologiques récentes. -D'autres chapitres ne changent guère d'intitulé mais évoluent significativement pour ce qui est de la définition des troubles qui les composent: les troubles disruptifs, du contrôle des impulsions et des conduites, les troubles bipolaires, la schizophrénie, les troubles dissociatifs, les dysfonctions sexuelles, les troubles à symptomatologie somatique. -Des trubles nouveaux apparaissent dans la classification principale: ex:le trouble de la communication sociale (pragmatique), le trouble disruptif avec dysrégulation émotionnelle, le sevrage du cannabis, les accès hyperphagiques, le sevrage de la caféine, la thésaurisation pathologique, la dermatillomanie, le jeu d'argent pathologique. -Une attention particulière est portée aux dimensions symptomatiques, qui transcendent parfois les frontières des catégories diagnostiques: ex: l'évaluation dimensionnelle de la sévérité des symptômes psychotiques par le clinicien, et le modèle alternatif pour des troubles de la personnalité dans la section III du DSM-5.

This book explains how radiology can be a powerful tool for establishing the diagnosis of many internal medicine diseases that are usually diagnosed on the basis of their laboratory or clinical presentation. It is organized in the classic fashion for internal medicine books, with eleven chapters covering the different internal medicine specialties. Within these chapters, more than 450 diseases are considered, some of which are rarely encountered but are nonetheless significant. For each disease, radiological and clinical features are displayed in images and high-quality digital medical illustrations, and those differential diagnoses are identified that can be ruled out by imaging alone. In addition, the pathophysiology underlying the radiological features is described, enabling the reader to understand why a particular sign is seen on MR images, CT scans, or plain radiographs. Head and neck pathology is increasingly becoming an attractive field for the new generations of pathologists, fully aware that in the era of specialization the information contained in the treatise of general surgical pathology, although extremely valuable to create the basis of their knowledge, does not provide in its full picture the specific knowledge required to meet current diagnostic and prognostic requirements.
