Californium-252 is a neutron emitter with a high specific activity, making it useful in a variety of applications, the most spectacular of which is in brachytherapy for cancer patients. Radiation oncology has exhausted nearly every option for treating radioresistant tumors by photon brachytherapy, and therefore new types of radiation need to be studied to improve the curability of cancer. This book provides an in-depth account of all aspects of radioembolization, a relatively novel technique based on the efficacy of radiotherapy for the treatment of liver tumors. In the past, the therapeutic radiation dose needed to treat a tumoral area has been sufficiently high to produce permanent damage to the nontumoral liver parenchyma. Radioembolization attempts to overcome this problem by combining embolization (intravascular deployment of particles) and brachytherapy (local administration of radiotherapy with the aid of a device, namely the particles themselves). The technique is based on improvements in percutaneous endovascular procedures which have allowed the deployment of small (25-35 µm) microspheres loaded with yttrium-90 that deliver high doses of beta-radiation specifically to the tumoral area. Performance of the procedure demands close familiarity with the hepatic vascular anatomy in order to ensure avoidance of vessels going to other territories. Furthermore, detailed knowledge of liver function, tumor volume, and liver vascular shunting is needed in order to calculate the appropriate therapeutic dose in each particular case. All of these topics are addressed in depth in this book, and initial clinical results are presented, showing that constant local control can be achieved with a reasonably low rate of complications. These promising results may lead to a progressive change in the therapeutic indication for radioembolization, and in some cases it could even become the therapy of choice.

Prostate brachytherapy has been the subject of heated debate among surgeons and the proponents of the various brachytherapy methods. This book reports the majority of lectures given during the NATO Advanced Study Institute ASI-982996, which was held at the European Scientific Institute of Archamps (ESI, Archamps – France) from November 15 to November 27, 2007. The ASI course was structured in two parts: the first was dedicated to what is often called “teletherapy”, i. e. radiotherapy with external beams, while the second focused on internal radiotherapy, also called “brachytherapy” or “curietherapy” in honour of Madame Curie who initiated the technique about a century ago. This ASI took place after the European School of Medical Physics, which devoted a 3 week period to medical imaging, a subject complementary to the topics of this book. Courses devoted to nuclear medicine and digital imaging techniques are collected in two volumes of the NATO Science Series entitled “Physics for Medical Imaging Applications” (ISBN 978-1-4020-5650-5) and “Molecular imaging: computer reconstruction and practice” (ISBN 978-1-4020- 8751-6). Every year in autumn ESI organises the European School of Medical Physics, which covers a large spectrum of topics ranging from Medical Imaging to Radiotherapy, over a period of 5 weeks. Thanks to the Cooperative Science and Technology sub-programme of the NATO Science Division, weeks four and five were replaced this year by the ASI course dedicated to “Physics of Modern Radiotherapy & Brachytherapy”. This allowed the participation of experts and students from 20 different countries, with diverse cultural background and professional experience.
