Writing a comprehensive scientific book about the cerebral palsy is a great challenge. Many different interventions are available for persons with CP. Increasingly, it is recognized that intervention needs to be evidence-based and family-centered. Related therapies can offer improvement in some cases but do not offer a cure. Lifelong re/habilitation (habilitation and rehabilitation) in person with cerebral palsy is the first part of this book which has four chapters about management in children and adults with cerebral palsy through the life span, providing support and services. Three chapters of the second part are exploring the new therapy options which could improve the family quality of life. Third part has two chapters about complementary therapies with new possibilities for the future.

Nowadays, cerebral palsy (CP) rehabilitation, along with medical and surgical interventions in children with CP, leads to better motor and postural control and can ensure ambulation and functional independence. In achieving these improvements, many modern practices may be used, such as comprehensive multidisciplinary assessment, clinical decision making, multilevel surgery, botulinum toxin applications, robotic ambulation applications, treadmill, and other walking aids to increase the quality and endurance of walking. Trainings are based on neurodevelopmental therapy, muscle training and strength applications, adaptive equipment and orthotics, communication, technological solves, and many others beyond the scope of this book. In the years of clinical and academic experiences, children with cerebral palsy have shown us that the world needs a book to give clinical knowledge to health professionals regarding these important issue. This book is an attempt to fulfill and to give “current steps” about CP. The book is intended for use by physicians, therapists, and allied health professionals who treat/rehabilitate children with CP. We focus on the recent concepts in the treatment of body and structure problems and describe the associated disability, providing suggestions for further reading. All authors presented the most frequently used and accepted treatment methods with scientifically proven efficacy and included references at the end of each chapter.

Questo volume riassume i principali elementi relativi alla funzione cammino nel bambino sano (sviluppo della funzione, meccanismi neurofisiologici e aspetti biomeccanici), prerequisiti fondamentali per poter comprendere i disturbi del cammino nel bambino affetto da paralisi cerebrale infantile (PCI). Vengono trattati i segni clinici tipici manifestati nel cammino dai bambini con PCI, la modalità per valutarli e interpretarli al fine di comprendere l’inquadramento diagnostico e prognostico relativo alla funzione in esame. Successivamente, vengono presentati gli ausili e le ortesi, e la loro possibile funzione all’interno del progetto riabilitativo. Il volume si conclude con una sezione di casi clinici, che descrive il processo di costruzione della seduta fisioterapica e le modalità di svolgimento attraverso esempi concreti di setting, esercizi terapeutici e proposte di compiti finalizzati. Cerebral palsy is a common pediatric problem and is the leading cause of childhood disability. It occurs at a rate of 3.6 cases per 1000 children, and represents a major social and psychological impact on both family and society. It is a group of disorders with movement difficulties being common for all affected patients. Its severity and extent are variable from one patient to another. Additionally, the impacts of cerebral palsy on daily activities, communications, and requirements are also variable. Recent advances in clinical research increase our knowledge and understanding of causal pathways, possible preventive measures, specific intervention strategies, and the value of new treatment modalities such as botulinum toxin and intrathecal baclofen in the management of cerebral palsy.

Questo volume analizza il concetto di fisioterapia nella accezione del “prendersi cura”, riprendendo le nozioni teorico-pratiche poste alla base della disciplina. Il principale tema affrontato è l’evoluzione e le possibili modalità di trattamento dei disturbi della postura seduta nel bambino con paralisi cerebrale infantile. Vengono affrontate le basi metodologiche su cui si basa il programma riabilitativo e messo a fuoco l’esercizio fisioterapico: dalla distinzione fra difetto e compenso alla definizione del problema principale, dall’individuazione degli obiettivi del trattamento alla costruzione della seduta fisioterapica. Nella seconda parte viene effettuata un’analisi della postura seduta nel bambino sano in relazione allo sviluppo neuromotorio, ai meccanismi neurofisiologici e agli aspetti biomeccanici. In seguito vengono illustrati i segni clinici tipici dell’organizzazione posturo-motoria dei bambini con PCI e viene descritta la modalità per valutarli e interpretarli al fine dell’inquadramento diagnostico e prognostico. La scelta degli ausili e delle ortesi e la loro possibile funzione all’interno del progetto riabilitativo completano l’argomento. Partendo dall’analisi del segno il testo affronta i principali difetti (impairment) che possono interessare l’arto inferiore nel bambino affetto da paralisi cerebrale infantile. Per ciascuna delle tre principali stazioni di movimento (piede, ginocchio e anca) vengono considerate le possibili cause del segno, le manovre di semeiotica clinica e strumentale (gait analysis) in grado di evidenziarle, le proposte di chirurgia funzionale più praticate capaci di modificarle. Cerebral palsy is a lifelong condition that affects the individual, family, and immediate community. Therefore, the goal of allowing the individual with cerebral palsy to live life with the least impact of the disability requires c- plex attention to the individual and the family. Furthermore, society needs to be sensitive and to accommodate individuals with disabilities by limiting architectural impediments and providing accessible public transportation and communication. The educational system provides the key means for helping the individual prepare to function in society to his or her maximum ability. In many ways, the medical care system probably has the least sign- icant role in preparing the child with cerebral palsy to function optimally in society. However, the medical care system is the place where parents first learn that their child has developmental issues outside the expected norm. It is almost universally the place where parents also expect the child to be made normal in our modern society. In earlier times, the parents would expect healing to possibly come from the doctor, but also they would place hope for healing in religion. As this belief in spiritual or miraculous healing has - creased, a significant font of hope has decreased for parents of young ch- dren with disabilities.

Il libro, frutto delle riflessioni sulla paralisi cerebrale infantile (PCI) che gli autori ed i loro collaboratori, medici e terapisti, hanno condotto negli ultimi anni, affronta i principali temi connessi alla valutazione delle funzioni adattive nelle forme spastiche della PCI (la definizione e le sue modifiche negli ultimi decenni, i nuovi orientamenti classificativi, l’eziopatogenesi, le correlazioni anatomo-funzionali, la semeiotica, i cosiddetti "disturbi associati": visivi, cognitivi, del comportamento). Lo scopo principale del volume non è quello di riportare lo "stato dell’arte" su questi argomenti, ma di offrire "appunti di viaggio" sui temi trattati, per suscitare riflessioni e confronti con l’esperienza dei lettori. Gli argomenti vengono trattati dagli autori secondoun’ottica fisiopatologica che guida la loro interpretazione della natura del difetto (diagnosi funzionale), dei problemi legati alla prognosi (come ipotesi di storia naturale) e alla riabilitazione (come modifica dell’architettura della funzione in senso adattivo).
