Inspired by witnessing and experiencing the changes in healthcare and its delivery over the past 50 years, Dr. Henry Buchwald observes and comments on the current state of healthcare in the United States. His narrative includes the history, the historical data, and personal experiences of a healthcare system that has moved away from caring, first and foremost, for patients. This expensive, impersonal system, he believes may not be in the best interest either of the nation or of the people it purports to heal. As the title suggests, it appears that healthcare has been turned upside down to serve the administrators of the system and away from its basic function of offering the best care for patients. With this basic principle in mind, the topics presented in this book provide and discuss healthcare statistics and alterations to the language of medicine. The chapters themselves examine the transformations to the medical school, the clinic and the office, the hospital, and the practice. Additional chapters discuss the role of the payers, public health research, as well as pandemics, including COVID-19, the advantages and disadvantages of socialized medicine, as well as the broken doctor/patient relationship. Finally, Dr. Buchwald offers thoughts on the areas in which future healthcare efforts can most fruitfully be expended. Analysing today's pervading administrative domination of essentially every facet of healthcare, Healthcare Upside Down thoughtfully considers the variety of ways in which we can turn the current healthcare system right-side up to serve those who should be the ultimate beneficiaries - all of us as patients, now and in the future.

This book highlights the reasons for an urgently needed revision of the current global healthcare setup, discusses the needed mindset for a future of health, and provides a comprehensive development toolset for disruption (and for the needed incremental innovations towards disruption). Today's biomedical and health innovation related research in universities encourages activities that lead to incremental innovations with a relatively low risk of failure. The healthcare industry on the other hand provides tools and devices for established healthcare providers to improve the diagnosis and therapy/ treatment of the patients' health problems. The patient is not in the center of healthcare provision however, and prevention and prediction are not core goals. The current health setup needs to be challenged and disrupted. Disruptions are coming from technologies or processes that lead to a significant (>10x) reduction in cost or price/ performance and that also come with new business models. The need for change, effects of exponential technologies, and the needed shift to prevention and to homecare for health democratization and patient empowerment will be discussed in detail in the first parts of the book. The subsequent sections address several innovation methods with a focus on a novel meta methodology named Purpose Launchpad Health. This is followed by a comprehensive discussion on health entrepreneurship activities and needs. The final section of the book addresses how to train students to become entrepreneurial health innovators, presenting successful curricula and examples of health incubation and accelerator setups. All of the innovation tools presented and used in this book are summarized in the final chapter to help the reader get started planning an entrepreneurial venture. Written by experts from academia and industry, the book covers important basics and best practices, as well as recent developments. Chapters are concise and enriched with key messages, learning objectives and real innovation examples to bridge theory and practice. This book aims to serve as a teaching base for health innovation design and to prepare for health-related entrepreneurial ventures. This open access edited book brings together new research on the mechanisms by which maternal and reproductive health policies are formed and implemented in diverse locales around the world, from global policy spaces to sites of practice. L'ingérence croissante des impératifs de protection de la santé sur le marché des biens et services, ainsi que l'émergence du consumérisme dans la sphère médicale témoignent du rapprochement réel de deux univers et des deux branches du droit qui les régissent : le droit de la santé et le droit de la consommation.Ainsi, la loi entend protéger le consommateur et le patient, et bien souvent ces deux catégories sont confondues d'où l'inéluctable rapprochement et complémentarité de ces deux droits spéciaux. L'ordre public, contenu par certaines consommations spéciales, a requis une adaptation du marché de la consommation et une rationalisation des opérations réalisées par le droit de la santé. Cet assainissement du marché de la consommation se prolonge par une supervision de l'opération de consommation jusqu'à sa réalisation.Le droit de la santé, telle une police du marché de la consommation, poursuit une action de sécurisation, de protection des consommateurs et de maîtrise des risques.Ces finalités justifieront l'élévation de restrictions au consumérisme, aux libertés du marché et l'imposition d'une modération dans la conclusion des actes de consommation. Aussi, le droit de la santé repose sur une approche collective et solidariste, sa propension au renouvellement et sa capacité d'expérimentation en font une source d'inspiration.
