Il y a de la valeur dans le secteur social et médico-social. Pas seulement des valeurs, conviction incantatoire qui ne pourrait s'appuyer sur la technicité d'un univers professionnel. Il y a des savoir-faire au sens de faire en situation avec des savoirs. Des savoir-faire producteurs de pratiques opératoires. Il ne s'agit pas d'y appliquer une théorie à la difficulté de la personne mais de rapprocher la théorie de sa difficulté singulière. Là se tient la clinique. L'ouvrage explore des notions anthropologiques et sociétales relatives au contrat, aux écrits, à la compensation, au projet de vie, etc. Puis il jette les bases d'une définition rigoureuse de trois domaines de savoir-faire : - la démarche d'accompagnement : observations, hypothèses, objectifs conçus sur le mode de l'interaction et non de la causalité ; - l'analyse clinique : distanciation des représentations sociales mais aussi des stéréotypes professionnels, identification de l'imaginaire institutionnel et du cadre symbolique, approche interdisciplinaire de la singularité d'une situation ; - les conditions d'élaboration de savoir-faire relatifs aux spécificités d'un public mais aussi structurels au travers de trois socialités : le groupe, l'équipe, l'institution. Les professionnels du secteur forment un corps de métier qui doit s'affirmer. C'est en construisant un professionnalisme qui se reconnaît dans la validité de ses savoir-faire que nous sommes utiles à ceux qui nous sont confiés.

"Insufficient attention has been paid to understanding what determines the performance of health workers and how they make labor market choices. This paper reports on findings from focus group discussions with both health workers and users of health services in Ethiopia, a country with some of the poorest health outcomes in the world. It describes performance problems identified by both health, users and health workers participating in the focus group discussions, including absenteeism and shirking, pilfering drugs and materials, informal health care provision and illicit charging, and corruption. The second part of the paper presents four structural reasons why these problems arise: (1) the ongoing transition from a health sector dominated by the public sector, toward a more mixed model; (2) the failure of government policies to keep pace with the transition toward a mixed model of service delivery; (3) weak accountability mechanisms and the erosion of professional norms in the health sector; and (4) the impact of HIV/AIDS. The discussions underline the need to base policies on a micro-analysis of how health workers make constrained choices, both in their career and in their day to day professional activities. Based on original fieldwork with outreach workers in HIV prevention it addresses issues of professionalism, emotion work and boundaries, integrating empirical insights with sociological theory.
