Parce l'image erronée d'une « réserve calorique» colle à la peau des lipides depuis trop d'années, le Dr François Mendy, pédiatre qui a consacré sa vie à la pratique de la médecine et la recherche clinique sur les acides gras, rend hommage dans cet ouvrage à la finesse des systèmes de régulation qui assurent un équilibre précis entre ces « matières grasses de la vie» . Au fil des chapitres, il replace les grandes découvertes, mais aussi les errements scientifiques, dans leur contexte historique. Il rappelle ainsi que la communauté scientifique a bien trop longtemps considéré les lipides comme de simples pourvoyeurs d'énergie, interchangeables avec les glucides et donc non obligatoires dans un régime alimentaire ; une vision erronée pourtant érigée en « dogme» qui empoisonnera la recherche jusque dans les années 1930, et même après ! Puis, il présente successivement les différents types de lipides (acides gras saturés, mono-insaturés, poly-insaturés), consacre un chapitre aux acides gras trans (l'occasion là encore de souligner les erreurs de la simplification qui a jeté l'opprobre sur tous les trans), un autre au cholestérol, le suivant aux mécanismes de régulation. Puis, le pédiatre propose une mise en application pratique à travers le suivi de l'incroyable croissance du fœtus puis de l'enfant, soutenue en grande partie par les lipides dans leur immense complexité d'action. Enfin, il dénonce 50 ans d'excès et de dérives médiatiques qui ont impacté le monde des lipides. et la santé des citoyens. Autant de chapitres dans lesquels le lecteur peut se plonger intégralement pour découvrir par exemple que les acides gras polyinsaturés sont aussi les précurseurs de nombreux médiateurs de l'inflammation, et participent à ce titre parfois à l'entretien parfois à l'extinction du « feu» des multiples pathologies inflammatoires (obésité, diabète, arthrite, etc.). et aux voies de recherche pour enrayer ces maladies. Il peut également « picorer» des réponses ici et là, chaque chapitre se terminant par un didactique résumé qui permet au lecteur d'en retenir l'essentiel.
