Abstract

Debulking in Cardiovascular Interventions and Revascularization Strategies: Between a Rock and the Heart aims to provide a comprehensive overview on the diagnostic and therapeutic aspects of resistant lesions as well as tailored debulking processes in revascularization strategies. Cardiovascular interventionalists and surgeons alike frequently encounter technical challenges and increased risk when attempting to diagnose and treat resistant cardiovascular lesions, bio-tissues and vessels. Calcified plaques, fibrotic or thrombotic lesions, total occlusions, degenerative plaques, complex lesions and vessels impose morphologic obstacles to the safe and effective delivery and deployment of balloons and stents. In such instances, a debulking strategy involving the actual drilling and extraction of the targeted plaque/bio-tissue constitutes a unique option that ensures safe and efficacious subsequent balloon dilatations and stenting.

Abstract

While the majority of patients with symptomatic coronary artery disease (CAD) achieve acceptable angina relief through medical therapy, coronary artery bypass grafting and percutaneous coronary intervention, a growing number are not candidates for these procedures.  Standard procedures are largely palliative, and symptom recurrence is relatively common, while the increase in obesity and type 2 diabetes may result in a potential explosion in severe, diffuse CAD.  As these trends continue, the population of patients who fail to achieve adequate symptomatic improvement through standard medical and revascularization strategies will grow exponentially. Therapeutic options for these patients are currently limited, but experimental and developing strategies are appearing, including transmyocardial laser revascularization, therapeutic angiogenesis, spinal cord stimulation, external counterpulsation, various novel medical therapies and complex percutaneous and surgical revascularization strategies. Coronary Artery Disease: New Approaches without Traditional Revascularization is a comprehensive and clinically oriented review of the epidemiology and basic science foundation for studies in this challenging group of patients. Abstract

The number of coronary artery bypass graft (CABG) operations has increased continually over the past decade. To date, roughly one-third of these are performed on women. Despite the tremendous progress made in cardiovascular surgery, not all patients profit equally from it, and the mortality rate of women still exceeds that of men by a factor of two. Studies conducted to investigate this marked gender difference have yielded conflicting results. In particular, adjustment for clinical risk factors seemed to diminish the difference, but could not fully explain it. Therefore psychosocial parameters, in particular depression and psychosocial stress, have gained increasing attention as predictors for both mortality and quality of life after CABG operation. Gender differences in these variables have long been observed in patients undergoing CABG: female patients are on average older, more often depressive and more often living alone than male patients. This book presents the very first German study to focus on these gender differences. By describing the gender-specific influence of psychological and clinical variables on the recovery process, this work fills an important gap in cardiovascular research. Its approach is interdisciplinary in the best sense. The study underlines the importance of psychosocial variables for both mortality and quality of life in the context of CABG. Thus, it may be seen as a basis for a better understanding and treatment of CABG patients.
