Abstract

The internet has transformed the world we live in, but it also poses new risks to our psychological well-being. This book provides an introduction to the issue of internet addiction, an increasingly common problem. All day, every day, we are connected to the internet, putting most people at some level of risk for internet addiction. Problematic internet use can take many forms, including overuse of social media and addictions to online shopping, gaming, or pornography. Such behaviors can cause anxiety, depression, sleep deprivation, loneliness, and physical health problems. People can lose their jobs and families, and in a few extreme cases, internet addiction has directly led to the death of the addicted individual or a child in their care. Abstract

The imperative to increase healthcare access, efficiency and effectiveness is nearly impossible to meet without reliance on technology. Telemedicine tools now include video, e-mail, text messaging, apps, and other mobile health modalities, deployed synchronously, asynchronously and in hybrid combinations to offer assessment, consultation, direct treatment, and integrated care. Within telemedicine, mental health is particularly well suited to technology platforms due to several inherent factors: provider shortages that are often more acute than in other specialties; the relatively reduced need for laboratory tests, imaging studies and physical examinations; the stigma still attached to mental illness; and diagnosis-specific obstacles that can complicate in person visits (e.g., pronounced fear in social anxiety disorder). The need to increase access, efficiency and effectiveness, combined with the relative ease of translating mental health interventions to technology-mediated delivery, have led researchers to explore various platforms, including: computerized cognitive behavioral therapy; online psychotherapy and online psychopharmacology; mobile therapy; virtual/augmented reality exposure therapy; serious games; and artificial intelligence. Yet, despite often positive efficacy data and an ever more technology-reliant lifestyle, the reach of telemental health remains relatively limited in comparison with its potential. We aim to capture the current status of digital and telecommunications technologies in mental health and the field’s future trends and directions. Recently published data—as well as newly introduced platforms—have put both the promise and challenges of telemental health in sharp focus, making this an opportune time to compile a collection of high-quality, evidence-based and diverse articles that touch on various aspects of this technology revolution. This Research Topic collects articles that examine digital mental health interventions across different platforms and modes of delivery. Abstract

Il concetto di psicopatologia web-mediata nasce dalla necessità di distinguere la dipendenza da internet, intesa come dipendenza patologica comportamentale, da dinamiche disfunzionali più complesse che coinvolgono bambini e adolescenti nativi digitali. Questo libro, che rappresenta un tentativo di comprensione di tali dinamiche, affonda le sue radici nell’esperienza clinica svolta, presso il Policlinico Gemelli di Roma, su pazienti con forme morbose che espandono i confini della clinica psichiatrica classica e che quindi lasciano pensare all’acquisizione di basi mentali diverse. La crescita inarrestabile delle relazioni internet-mediate, destinate nel tempo a prevalere sui contatti “dal vivo”, ha moltiplicato le possibilità di comunicare tra i giovani e complicato invece le relazioni con la generazione precedente, quella dei genitori, i quali spesso sull’argomento risultano paradossalmente meno competenti dei figli. Il volume esplora quindi la complessità dei quadri psicopatologici che ne scaturiscono, analizzando la dipendenza da internet nel contesto allargato di altre dipendenze comportamentali, come quella da gioco d’azzardo (online gambling) e varie forme di perversione delle condotte sessuali (cybersex). Dopo la descrizione delle trasformazioni socioculturali che l’era digitale ha generato nel modo di vivere il tempo e lo spazio e l’emergere di gravi condotte di ritiro sociale negli adolescenti (fenomeno noto in Giappone come hikikomori), viene affrontato il concetto di dissociazione come nucleo fondante della maggior parte delle manifestazioni cliniche internet-correlate, con la proposta di trattamenti specifici. La psicopatologia web-mediata coinvolge quindi molte aree cognitive, come la percezione, l’attenzione, la memoria, il pensiero e l’istintualità, e dà origine a un nuovo profilo cognitivo, frutto di un diverso modo di apprendere, comunicare e pensare.

Abstract

This book presents a new, evidence-based cognitive behavioral intervention for the prevention and treatment of Internet addiction in adolescents. It provides a comprehensive overview of the current state of research regarding phenomenology, diagnostics, epidemiology, etiology, and treatment and prevention of Internet addiction as a new behavioral addiction. The book is divided into two sections. The first part of the book explores various bio-psycho-social factors that contribute to the development and maintenance of symptoms in young individuals. Chapters in the second part of the book discuss the PROTECT intervention to reduce Internet addiction in adolescents. PROTECT aims to modify risk factors and maintenance factors, specifically, boredom and motivational problems, procrastination and performance anxiety, social anxiety and maladaptive emotion regulation. The PROTECT intervention is a low-intensity approach which uses comprehensive case examples in order to increase cognitive dissonance and treatment motivation. In addition, PROTECT contains cognitive behavioral intervention techniques such as psychoeducation, behavior activation, cognitive restructuring, problem solving and emotion regulation.
