A simple, straightforward, and foolproof proposal for universal health insurance from a noted economist.The shocking statistic is that forty-seven million Americans have no health insurance. When uninsured Americans go to the emergency room for treatment, however, they do receive care, and a bill. Many hospitals now require uninsured patients to put their treatment on a credit card which can saddle a low-income household with unpayably high balances that can lead to personal bankruptcy. Why don't these people just buy health insurance? Because the cost of coverage that doesn't come through an employer is more than many low- and middle-income households make in a year. Meanwhile, rising healthcare costs for employees are driving many businesses under. As for government-supplied health care, ever higher costs and added benefits (for example, Part D, Medicare's new prescription drug coverage) make both Medicare and Medicaid impossible to sustain fiscally; benefits grow faster than the national per-capita income. It's obvious the system is broken. What can we do?In The Healthcare Fix, economist Laurence Kotlikoff proposes a simple, straightforward approach to the problem that would create one system that works for everyone and secure America's fiscal and economic future. Kotlikoff's proposed Medical Security System is not the "socialized medicine" so feared by Republicans and libertarians; it's a plan for universal health insurance. Waarom geneesmiddelen zo duur zijn…Zocor is een veel verkochte pil tegen verhoogd cholesterol. Het staat samen met twee andere cholesterolverlagers in de topvijf van medicijnen die de ziekteverzekering het meeste kosten. Dokter Dirk Van Duppen toonde aan dat de terugbetalingscriteria voor cholesterolverlagers van het Belgische RIZIV vele miljoenen opleverden voor Merck Sharp & Dome (MSD), de firma die Zocor op de markt brengt. Want 270.000 Belgische patiënten zouden onnodig Zocor slikken.Ontsteltenis bij farmaceutische firma's en bij minister Vandenbroucke. Applaus bij de patiënten, de dokters en apothekers, de ziekenfondsen. Ondanks het gelobby haalde david Van Duppen z'n slag thuis tegen goliath (MSD). De cholesterolverlagende statines werden alvast veel goedkoper. De firma Pfizer haalt met Lipotor, de duurste (een andere) cholesterolverlager, een jaaromzet van 85 miljoen euro, al kon geen enkele deugdelijke studie de werkzaamheid van Lipitor afdoende bewijzen, wat wel het geval is voor andere statines.In deze cholesteroloorlog kwam dokter Van Duppen tot de vaststelling dat patiënten álle medicijnen veel te duur betalen. Hij bond de kat de bel aan over de dure regeling in België voor de terugbetaling van medicijnen. Hij vond in Nieuw-Zeeland een veel doorzichtiger en goedkoper systeem: een systeem van openbare aanbesteding waarbij alleen het beste medicijn tegen de laagste prijs op de markt wordt toegestaan en ook volledig terugbetaald. Resultaat: de medicijnen zijn er 50% tot 90% goedkoper dan in België. Geïnspireerd door dat systeem lanceerde dokter Van Duppen een wetsvoorstel om een forse prijsdaling af te dwingen. Wordt vervolgd!

Abstract

The World Health Report 2003: Shaping the Future: A Vision for Global Health predicts that major global health improvements will be achieved in the next few years. Dr Jong-Wook Lee, Director-General of WHO, describes WHO's ambition ''to help all people, but especially the poorest among them, to shape a better future''. The report advocates a new drive for equitable health improvement as a vital part of global efforts for justice and security. Drawing on lessons of the past, in particular the Health for All movement that was launched 25 years ago, this year's report focuses on the major health gains that are possible in the coming years and shows how WHO and the global health community can achieve these objectives. Against a backdrop of the ambitious targets of the Millennium Development Goals, The World Health Report 2003 presents a comprehensive vision encompassing an analysis of today's major health challenges and an action programme for global health improvement. The section on infections singles out for special attention HIV/AIDS, poliomyelitis - whose elimination is within reach - and new infections including SARS. Another important section deals with the health of women and children. The focus of the noncommunicable diseases section is on road traffic injuries and the continuing work on tobacco control. In order to achieve the health improvements that are within their grasp, countries need well-functioning health systems. The report shows how systems can be strengthened, combining both primary care facilities and higher-level services linked by an efficient and integrated referral network. In this, as in all its work, WHO renews its commitment to working with countries in responding to national and local health challenges.

Abstract

Het werk vangt aan met een algemeen overzicht van zowel de kenmerken van het Belgische gezondheidszorgsysteem, als van het ontstaan van de belangrijkste actoren. Het aanbod in de gezondheidszorg, de werking, de programmatie, de erkenning en de financiering van ziekenhuizen en andere instellingen zoals rusthuizen en rust- en verzorgingsinstellingen krijgen de nodige aandacht. Het organiseren van de gezondheidszorg veronderstelt een juridisch kader. De auteurs gaan na wie bevoegd is om op Europees, federaal en gemeenschapsniveau regelgeving inzake de gezondheidszorg uit te werken. Vermits zowel het behandelen van patiënten als het voeren van een beleid en onderzoek niet mogelijk is zonder het verwerken van gezondheidsgegevens, geven de auteurs in een aparte titel de grote principes weer van het verwerken van gezondheidsgegevens. De financiering van de gezondheidszorg met een beschrijving van de structuur van ziekenfondsen en het RIZIV en met een overzicht van financieringstechnieken krijgt de nodige aandacht in dit boek. Na een titel waarin de problematiek van de kwaliteit van de gezondheidszorg en de belangrijkste adviesorganen uit de sector aan bod komen, besteden de auteurs aandacht aan het onderzoek naar en de productie en verdeling van geneesmiddelen. Abstract

As America's population ages, economic research related to the elderly becomes increasingly important to public policy. Frontiers in the Economics in Aging directs attention to four topics: the role of retirement accounts, such as IRAs and 401(k)s in personal saving; the economics of health care; new advances in research methodology; and aging in relation to inequality. Some of the issues analyzed within these topics are the implications of rising personal retirement saving in recent years, how health and health insurance affect labor supply, and the effects of pensions on the distribution of wealth. David Wise's lucid introduction provides an overview of each paper. In addition to this book's appeal for specialists and microeconomists, it offers immediately practical ideas and methods for shaping public policy. In fact, one of the papers in this volume, "The Taxation of Pensions: A Shelter Can Become a Trap," helped to spur new legislation that reformed laws on pension distribution.

Abstract

This paper analyzes the macroeconomics of HIV/AIDS. The paper highlights that the mortality and morbidity associated with AIDS make it unlike most other types of sickness and disease. The paper describes the most common approaches used in accounting for growth in the context of an HIV/AIDS epidemic. The impact of HIV/AIDS on education and the accumulation of human capital is discussed. The paper also discusses the impact of HIV/AIDS on the public sector, and elaborates certain demographic events specific to the HIV/AIDS pandemic.
