The American Cancer Society recently estimated that about 45,000 new cases of thyroid cancer will be diagnosed in the United States, with three-quarters occurring in women. The overall 5-year survival rate is about 97%, making it one of the least lethal cancers. We are experiencing an epidemic of well-differentiated thyroid cancer, in part due to the widespread use of imaging modalities that detect thyroid nodules and microcarcinomas. Concurrently, there have been a number of recent advances in surgical treatment, as well as diagnostic modalities that allow us to detect small amounts of residual local and metastatic disease. Additionally, a reexamination of past treatment regimens has led to new recommendations regarding the use of radioactive iodine, and to new therapeutic options, such as targeted therapy which have supplanted the use of more toxic chemotherapy for metastatic cancer. Creating clinical guidelines is a modern trend. Published studies pertaining to a given theme are collected, their credibility evaluated, and then treatment options in the form of evidence-based guidelines are offered. There are a number of guidelines for the treatment of thyroid tumors that have established positions in clinical practice in North America and in Western European countries. In Japan, however, where radioisotope facilities are of limited availability, treatment plans for differentiated thyroid cancer differ considerably from those of America and Europe, and the associated clinical guidelines need modification before they can be adopted. In addition, although thyroid tumor is a common disease in endocrine practice, its management can differ even among specialists. Thus, a Japanese clinical guideline for the treatment of thyroid tumor was desired by many clinicians. As a combination of evidence-based and consensus-based guidelines for the treatment of thyroid tumor, this book offers alternatives to conventional approaches in the West. Ultimately, the authors hope the guideline will lead to the best possible treatment for patients all over the world in the not-distant future.

Thyroid fine needle aspiration is one of the most commonly performed cytologic procedures and is the standard diagnostic method for managing the patient with a thyroid nodule. Until now, there has been no uniform system for reporting the results of thyroid fine needle aspiration (FNA) in the U.S. The terminology described in this book is an important advance in the field of cytopathology. It represents the results of the multidisciplinary National Cancer Institute Thyroid Fine Needle Aspiration State of The Science Conference, held in Bethesda, MD in October, 2007. The Bethesda System for Reporting Thyroid Cytopathology will provide the reader with a unified approach to diagnosing and reporting thyroid FNA interpretations. It will serve as a reference guide not just for pathologists, but also endocrinologists, surgeons, and radiologists.
