Abstract

Since George Rosen’s comprehensive History of Public Health, first published in 1956, there has been no internationally comparative survey of the subject. Over the past three decades or so, however, research in this field has expanded rapidly, especially with regard to the history of disease and social order and public health politics and the state. Most of these studies have been highly scholarly and specialised and often dealing with only one aspect of public health in any one national context. The essays here examine the road history of public health in different national contexts in order to provide a work of comparative reference that could be used as a teaching aid. The book focuses on whether the construction of a public health system is an inherent characteristic of the managerial function of modern political systems. Thus, each essay traces the steps leading to the growth of health government in various nations, examining the specific conflicts and contradictions which each incurred. As a result the volume highlights the need for further comparative analysis of public health systems as a highly fruitful topic for future study.

The co-operative spirit of citizens in twentieth-century Saskatchewan nurtured innovation in health care and health policy. 36 Steps on the Road to Medicare showcases the decisions that led to the province's medicare system - the forerunner of Canadian health care. Stuart Houston and Merle Massie document the range of Saskatchewan leadership on Canadian, North American, and world stages: municipal doctors and municipal hospitals, the first Red Cross Outpost Hospital in the British Empire, tuberculosis diagnosis and treatment, a successful pilot comprehensive regional health care plan, government-sponsored cancer clinics, innovative LSD and patient-oriented treatment for psychoses, the first full-time cancer physicist in Canada, and the world's first concerted clinical use of the betatron and Cobalt-60 in cancer treatment. They show how North America's first social-democratic government, the Co-operative Commonwealth Federation - elected in 1944 and led by the incomparable Tommy Douglas - created the blueprint for comprehensive health care and how sequential steps on the road to medicare were implemented quickly and within budget. When federal support for national hospitalization became available, Saskatchewan could afford to initiate medicare in 1962. Other Canadian provinces soon followed Saskatchewan's lead. "In this edition of Canada: State of the Federation, contributors consider whether and to what degree the relationship between the central government and the provincial and territorial governments has changed in the past decade. The authors address three overarching questions. First, is the power base changing in Canada? If so, how are governments responding? Second, what are the implications of the changing environment for the relationships between governments? And third, are there underlying forces - such as economic or technological change, or demands for citizen engagement - that are pushing some provinces and regions to become more assertive in the global environment? The papers are organized into four categories: those that identify and analyze the changing federal environment; those concerned with the implications of the 2011 federal election; those that deal with health policy and economic federalism; and those that explore the growing importance of the North and the changing dynamics among the provinces and the federal government.
