Abstract

Infants are not just small adults; they have a complex set of nutrient requirements and interactions to account for the high metabolic rate, growth, immunological and cognitive development, etc. Each year there are 138 million births, including 30 million from the least developed countries, where health is most at risk. Nutrition during infancy establishes growth patterns and development which moderate short term and long term health and life expectancy. There is continuing progress in understanding the nutrient composition of breastmilk and the importance nutritious and timely complementary foods. Nutrients at particular risk in the first years of life are iron, iodine, and vitamins A and D. There are still gaps in our knowledge on the influence of maternal diet, body composition and nutritional status on breastmilk nutrients. Interaction of nutrients with the human microbiome and gastro-intestinal tract hormonal secretion are developing fields. While breastmilk remains the “gold standard of pediatric nutrition”, the provisions of nutrients providing for optimal health in infant formulae, including nutrients and probiotics remains an area of research. The assessment of body composition for research and clinical practice has progressed rapidly and is important in understanding later obesity. After six months of age, there are new issues of the interaction of weaning foods and later family foods with the maturing gastro-intestinal tract to promote health and growth. Infant nutrition is made more difficult (and interesting) by the many cultural beliefs related to feeding practices. Developments in all of these fields of research into pediatric nutrition will be explored in this special issue together with state of the art reviews.

Abstract

Breast-Feeding: Early Influences on Later Health is a new book which draws together areas of research in early lifel programming of adult health, with a unique focus on the post-natal period in terms of early life programming particularly the extent to which differences in infant feeding practices can lay an indelible imprint on metabolism and behaviour, and hence affect later function and risk of disease. This is an area where there is much less information currently available than there is for fetal programming, and the book raises many new questions and highlights numerous areas where further research is needed. The book chapters are arranged in three core sections: Chapters 1-4 lay down some of the basic biology of early life development; Chapters 5-9 examine how breast-milk and breast-feeding might ‘programme’ these processes by acting as modulators of development; Chapters 10-17 examine the epidemiological evidence that such effects do indeed exist. In addition the book includes unique chapters on the Evolution of human lactation and complementary feeding, The Macy-György Prize Lecture ‘My Milky Way’, updates on HIV and Breast-Feeding and on Early breastfeeding cessation and infant mortality in low-income countries, and measuring trace immune factors in human milk, all important topics that have such a critical impact on child health and survival in many countries.

Abstract

In pregnancy, maternal nutrition sustains and nourishes the developing child. Imbalances in either the direction of nutritional excess or deficiency can have adverse consequences for child health. In addition, more research now suggests that good pregnancy nutrition influences child health beyond pregnancy and delivery. This includes modifying the risk of child health outcomes as they enter childhood and adulthood through influences on placental development, hormonal pathways, and organ structure and function. Poor pregnancy nutrition may also compromise maternal health during pregnancy, which may have long-term consequences for women’s health. Understanding the biological and social mechanisms operating during pregnancy can help in the design of better clinical and public health interventions. This Special Issue on “The Role of Pregnancy Nutrition in Maternal and Offspring Health” includes etiological and mechanistic studies of pregnancy nutrition with short- and long-term maternal and child health outcomes, including original research, narrative reviews, and systematic reviews and meta-analyses. Together, this body of work provides important insights into the influence of dietary patterns, food groups, and nutrients on pregnancy outcomes, and long-term neurodevelopmental, respiratory, and metabolic health in the children. It also highlights nutritional consequences for specific groups of women, including those with pregnancy complications and eating disorders.

Abstract

In the nineteenth century, infants were commonly breast-fed; by the middle of the twentieth century, women typically bottle-fed their babies on the advice of their doctors. In this book, Rima D. Apple discloses and analyzes the complex interactions of science, medicine, economics, and culture that underlie this dramatic shift in infant-care practices and women's lives. As infant feeding became the keystone of the emerging specialty of pediatrics in the twentieth century, the manufacture of infant food became a lucrative industry. More and more mothers reported difficulty in nursing their babies. While physicians were establishing themselves and the scientific experts and the infant-food industry was hawking the scientific bases of their products, women embraced "scientific motherhood," believing that science could shape child care practices. The commercialization and medicalization of infant care established an environment that made bottle feeding not only less feared by many mothers, but indeed "natural" and "necessary." Focusing on the history of infant feeding, this book clarifies the major elements involved in the complex and sometimes contradictory interaction between women and the medical profession, revealing much about the changing roles of mothers and physicians in American society.
