Abstract

Die Transition chronisch kranker Jugendlicher in die Erwachsenenmedizin ist ein weltweit diskutiertes Problem der medizinischen Versorgung, das trotz vielfältiger wissenschaftlicher Untersuchungen und Initiativen erst unzureichend gelöst ist. Auch in Deutschland ist die Transition bisher nicht einheitlich geregelt. Sie betrifft eine große Zahl von jungen Menschen mit sehr unterschiedlichen Erkrankungen. Das Berliner TransitionsProgramm (BTP) ist ein sektorübergreifendes Strukturprogramm, das Jugendliche mit unterschiedlichen Erkrankungen durch ein professionelles Fallmanagement begleitet. Es ist das erste seiner Art und dient als Vorlage für ein bundesweit einheitliches, durch enstprechende Fachgesellschaften unterstütztes Transitionskonzept. Es wir durch eine wachsende Zahl von Kostenträgern vergütet.
