Atopic dermatitis or eczema is an increasingly common skin disease, but its distribution, frequency and underlying causes have not yet been systematically reviewed in depth: this is the very first book to look at the epidemiology of atopic dermatitis, its prevalence and possible causes. Atopic Dermatitis is a common disease characterized by inflamed, itching and dry skin. This relapsing allergic disorder has complex etiology and shows a remarkably high clinical heterogeneity which complicates the diagnosis and clinical management. This book is divided into 4 sections. The first section (Disease Etiology) describes some of the physiological mechanisms underlying Atopic Dermatitis, including alterations in the immune system and the skin-barrier function. The important role of host-microorganism interactions on the pathophysiology of Atopic Dermatitis is discussed in the second section (Microorganisms in Atopic Dermatitis). An overview of the clinical diagnostic criteria and the disease management protocols commonly used is given in the third section (Diagnosis and Clinical Management). The last section (New Treatments) describes new therapeutic approaches that are not widely used but are currently being studied due to preliminary evidence showing a clinical benefit for Atopic Dermatitis.

L’esigenza di chiarire i concetti attuali dell’atopia, che per troppo tempo hanno oscillato tra una primitiva visione squisitamente clinica ed una successiva unicamente immunologica, era matura da tempo. L’avere compreso che il ruolo delle immunoglobuline E non è esaustivo ed avere accettato la salomonica divisione tra atopia intrinseca ed estrinseca, sono stati i primi due passi fondamentali. Le recenti scoperte del ruolo dell’immunità innata, e quindi dei peptidi antimicrobici, hanno aperto dei fondamentali spazi di comprensione nell’atopia e non solo. Quello che ora appare chiaro è che l’atopia non è la malattia di un organo ma piuttosto un difetto delle membrane che delimitano il nostro "self". Il fatto che queste membrane si chiamino pelle, intestino, congiuntiva, bronchi o quant’altro ha artificialmente frammentato la comprensione ed il trattamento di un fenomeno unitario. Questo libro vuole cercare di fare una sintesi delle diverse esperienze ma soprattutto essere uno stimolo a far sì che la medicina si focalizzi costruttivamente sullo stesso bersaglio. Se così sarà, non assisteremo più a"guerre di religione" tra scienziati e medici pratici o, peggio ancora, tra i differenti specialisti che si contendono il paziente. Se l’atopia, come noi crediamo, è un problema globale, esso può essere adeguatamente affrontato solo in un contesto pluridisciplinare e in clima collaborativo, non solo tra medico e medico ma anche tra medico e paziente.

Atopic eczema (atopic dermatitis, neurodermitis diffusa, endogenous eczema) is one of the most common skin diseases of our time and is still increasing in prevalence dramatically all over the world. The reasons for this increase are not known. It occurs at any age, not only in childhood, approximately 2/3 of children affected will suffer from this disease when they are grown-up. The chapters in this volume provide an account of the present knowledge and understanding of Foot and Mouth disease virus pathogenesis and global epidemiology, the detailed structure of the virus itself and the properties of its RNA genome. La consapevolezza che l’atopia sia l’espressione di una più facile suscettibilità della cute e delle mucose (oculari, respiratorie, digestive) agli agenti esterni in senso lato, porta alla logica conseguenza che i professionisti della salute che se ne debbano occupare siano vari e necessariamente coordinati. La nozione di atopia allarga il concetto tradizionale di allergia, perché in molti pazienti atopici non sono dimostrabili le classiche alterazioni immunologiche dell’allergia. Questi due fatti hanno quindi portato alla creazione di Dipartimenti dell’Atopia nelle strutture sanitarie di vari paesi. Lo scopo di questa pubblicazione è quello di "tradurre" e schematizzare gli argomenti svolti in maniera accademica nella prima edizione della Scuola dell’atopia e che si sono condensati nel testo omonimo. Questo libro, invece, si propone di essere uno strumento sintetico e di rapida consultazione, destinato a chi si confronta con questi pazienti nel quotidiano: medico, pediatra di base, infermiere o farmacista. Il libro comprende un atlante fotografico cronologico ed annotato delle principali manifestazioni dell’atopia cui segue la sezione degli algoritmi commentati su ciascuno dei principali capitoli. La terza parte, infine, offre un elenco critico dei "prodotti" per l’atopia disponibili in Italia ed include, oltre ai farmaci topici, una selezione dei dermocosmetici utili nella gestione dei questi pazienti.
