Abstract

Dépression et démence ne seraient-elles qu’une seule maladie, comme le double visage de Janus, qu’Ovide identifiait au Chaos ? Longtemps présentées comme un diagnostic différentiel à envisager lors des troubles mnésiques débutants, ces deux affections intriquées demeurent une énigme pour le corps médical confronté au déclin de la personne âgée. Égarant le diagnostic, leur étiopathogénie demeure incertaine à plus d’un égard. Sont-elles des sœurs siamoises issues d’une pathologie organique unique préexistante ou la conséquence l’une de l’autre : la détresse du dément qui prend conscience de son invalidité ou la dépression à ce point déstructurante qu’elle débouche sur la folie ? Les arguments pour chacune de ces hypothèses seront avancés avec toutes les précautions d’usage, sans prétendre imposer au lecteur une réponse univoque à la fi n du parcours.

"Wandering the Wards provides a detailed and unflinching ethnographic examination of life within the contemporary hospital. It reveals the institutional and ward cultures that inform the organisation and delivery of everyday care for one of the largest populations within them: people living with dementia who require urgent unscheduled hospital care. Drawing on five years of research embedded in acute wards in the UK, the authors follow people living with dementia through their admission, shadowing hospital staff as they interact with them during and across shifts. Abstract

With an aging population suffering from a high level of cardiac and cerebrovascular disease, it has become increasingly clear that the time may be ripe for a deeper understanding of vascular disease (VaD). In Vascular Dementia: Cerebrovascular Mechanisms and Clinical Management, a panel of multidisciplinary experts summarizes our current understanding of the biological and clinical aspects of the condition. The authors describe the basic mechanisms associated with aging and cerebrovascular disease that may play an important role in the development of VaD, and identify its impact on cognitive status, psychiatric health, and the ability of patients to complete the tasks of daily living. Additional chapters address the pharmacological management of vascular dementia and the use of neuroimaging methods to investigate it, with particular attention devoted to both functional and structural imaging techniques. The complex relationship between VaD and Alzheimer's disease (AD) is fully explored, with chapters on how these processes may interact and how one disease may lower the threshold for clinical expression of the other. The authors also consider the impact of VaD on the perceived quality of life of patients and caregivers, two issues rarely discussed in the scientific community. Abstract

How can we provide good spatial orientation for people with dementia? More than 35 million people worldwide currently live with dementia, and the number will double by 2050 according to professional forecasts. This development also presents a unique challenge to architecture and urban planning, because dementia is associated with problems in orientation and, hence, is closely related to spatial contexts. Questions such as these are addressed extensively in a selection of professional essays that also carve out specific requirements for various disciplines.
