Abstract

"Kidney transplantation has become a routine therapeutic option for patients with end stage kidney disease globally. Over 16,000 to 19,000 kidney transplants are performed annually in the United States. While transplant nephrologists care for transplant recipients and living donors in the perioperative period, general nephrologists are expected to care for these patients after three months following their transplants. The management of immunosuppressive therapy and complications associated with the transplant process is complex and requires an in-depth knowledge of transplant medications, drug interactions, both acute and long-term complications, among many other transplant and non-transplant related problems. Selection and referral of potential living donors are other processes that are vital to kidney transplantation and require the involvement of general nephrologists. Most general nephrologists, however, are not well-trained in kidney transplantation and/or do not care for enough kidney transplant patients to gain adequate experience to provide patients with the best possible care"--Provided by publisher.

Abstract

This Special Issue in renal transplantation covers a variety of clinical and research areas in kidney transplantation. The recent decade is associated with an ongoing shortage of organs for transplantation with efforts to increase the organ pool with DCDs and extended criteria donors. However, with the increasing success rate of kidney transplants, there is also a growth in the candidate list because of removal of the age barrier and transplantation of high risk patients with other comorbidities. The future seems promising with the development of innovative non-invasive technologies introducing biomarkers for diagnosis of rejection and ischemic reperfusion injury, use of cell therapy for tolerance induction, development of artificial organs, and overcoming immune and non-immune barriers in xenotransplantation. This Special Issue will touch some of these topics that are in the frontiers of the modern era of kidney transplantation.

Abstract

There are many obstacles in kidney transplantation. For the transplant team, there is the balance between immunosuppression to aid in the recipient's tolerance of the allograft and the infection risk of a suppressed immune system. These potential long term complications of kidney transplantation are relatively well known, but there are many other complications that patients and families do not consider when preparing themselves for a kidney transplant. Although the benefits of attempting a kidney transplant far outweigh downfalls of the long term sequelae, kidney transplantation is by no means a benign procedure. It is the hope of these authors that the reader will leave with a sense of understanding towards the kidney recipients.
