Abstract

This first-of-its-kind title addresses the failures of an often fragmented healthcare system in managing vulnerable patients with multiple, chronic, co-morbid conditions -- patients who are frequently unresponsive to the methods and approaches used to treat other patients with conditions that are less complicated. The book emphasizes a holistic evaluation to patient care that looks at the whole patient, providing comprehensive formulations that describe the interacting problems that afflict the patient, including elements that are barriers to effective treatment of active medical problems and barriers to recovery. The book begins by defining integrated care, discussing the types of patients who benefit from this approach and some of the models of care, including financing, barriers to acceptance, and advocacy for patients. The second section discusses the structural elements of integrated care, including the building of a team approach, issues of leadership, and role definition, as well as the authors’ experiences in overcoming some of the problems. In the remaining sections, the book discusses major complicating features of the patients seen in integrative care settings, including a description of the kinds of problems, a model for formulation of patient cases, and successful approaches to treatment of these problems. Abstract

Pour la plupart des Français qui l’ont vécue, la période de l’Occupation évoque en premier lieu l’immense difficulté à vivre au quotidien. La pénurie des produits de première nécessité, soumis à un rationnement de plus en plus sévère, oblige nombre d’entre eux à élaborer de véritables stratégies de survie. La faim, dont le spectre avait cessé de hanter les imaginaires, redevient une expérience largement partagée et structure bien des existences recentrées sur la satisfaction au jour le jour des besoins vitaux. Or si les historiens admettent que les difficultés de ravitaillement et les « privations » ont joué un grand rôle dans la montée des oppositions au régime de Vichy, ils ne se sont guère intéressés aux victimes de la « famine lente ». Difficiles à dénombrer, celles-ci ne sont pas prises en compte dans les statistiques des victimes du conflit. C’est à cette lacune que remédie cet ouvrage qui tente d’identifier tous ceux qui, dans la France occupée, sont morts de faim, de froid mais aussi de pathologies afférentes à la dénutrition (tuberculose en particulier). Parce que, trop isolés ou trop fragiles, ils n’avaient pas les moyens financiers, physiques et/ou psychiques de remporter cette épuisante « course aux calories » à laquelle il fallait se livrer pour survivre. Les auteurs se penchent sur le sort des « pensionnaires » de collectivités, hospitalières (hospices de vieillards, hôpitaux généraux, sanatoriums) et non-hospitalières (prisons, camps d’internement), en mettant l’accent sur celui des aliénés internés dans les hôpitaux psychiatriques qui ont payé le plus lourd tribut à la sousalimentation. Ils analysent également le devenir des groupes socialement et biologiquement les plus vulnérables de la population des grandes villes et les ripostes, sélectives, mises en oeuvre par les autorités pour atténuer les effets des restrictions sur la santé publique.

Abstract

A study of the importance of self-trust for women's autonomy in reproductive health. The power of new medical technologies, the cultural authority of physicians, and the gendered power dynamics of many patient-physician relationships can all inhibit women's reproductive freedom. Often these factors interfere with women's ability to trust themselves to choose and act in ways that are consistent with their own goals and values. In this book Carolyn McLeod introduces to the reproductive ethics literature the idea that in reproductive health care women's self-trust can be undermined in ways that threaten their autonomy. Understanding the importance of self-trust for autonomy, McLeod argues, is crucial to understanding the limits on women's reproductive freedom. McLeod brings feminist insights in philosophical moral psychology to reproductive ethics, and to health-care ethics more broadly. She identifies the social environments in which self-trust is formed and encouraged. She also shows how women's experiences of reproductive health care can enrich our understanding of self-trust and autonomy as philosophical concepts. The book's theoretical components are grounded in women's concrete experiences. The cases discussed, which involve miscarriage, infertility treatment, and prenatal diagnosis, show that what many women feel toward themselves in reproductive contexts is analogous to what we feel toward others when we trust or distrust them. McLeod also discusses what health-care providers can do to minimize the barriers to women's self-trust in reproductive health care, and why they have a duty to do so as part of their larger duty to respect patient autonomy.
