Wells, Jr., MD Professor of Surgery Duke University Medical Center, Durham, NC According to the American Cancer Society, about 1 million persons in the United States will be diagnosed with solid cancer every year. About 50% of them will be potentially cured by surgery. The other half of the population may develop metastatic cancer. To date, there is no systemic treatment available to cure metastatic cancer. Therefore, cancer clinical trials are critical to evaluate reliable treatment modalities against metastatic cancer. Likewise, adjuvant trials are needed to prevent high risk patients from developing recurrence following definitive surgical resection of their cancer. Only about 3-5% of the adult cancer patients are enrolled in clinical trials. When the pediatric cancer clinical trial groups were able to accrue over 40 percent of eligible patients to their clinical trials, significant impact was made on the survival of these pediatric cancer patients. However, multiple barriers exist to block adult cancer patients from entering into clinical protocols. The ever-changing regulations for clinical trials and the ethical dilemma of treating cancer patients as subjects have made it ever so difficult for the principal investigators to conduct clinical research, especially when they are often ill-informed of the complex nature of the regulations and over-worked. Therefore, cancer clinical trials are at a critical junction. The objective of this book is to bring the issues of cancer clinical trials into focus so that proactive strategies may be developed to make such trials more user-friendly. Sentinel lymph node (SLN) procedures have opened a window of opportunity for the study of micrometastasis. In eighty percent (80%) of metastasis there lies an orderly pattern of progression via the lymphatic network, while 20% of the time systemic metastasis occurs, bypassing the lymphatic system. During the past two decades, significant progress has been achieved in understanding the anatomical, functional, cellular and molecular aspects of the lymphovascular system and the metastasis process. · Molecular imaging advances help to localize early cancers more precisely. · Current status of the immune responses in the draining lymph nodes against cancer is summarized. · New paradigms of early cancer growth, proliferation, overcoming apoptosis are exploited in the development of anticancer treatment. Abstract

There has been a wealth of recent research on the complex changes involved in bread making and how they influence the many traits consumers use to define quality. Bread making: improving quality sums up this key research and what it means for improved process control and a better, more consistent product. After an introductory review of bread making as a whole part one discusses wheat and flour quality. Chapter 3 summarises current research on the structure of wheat, providing the context for chapters on wheat proteins (chapters 5 and 6) and starch (chapter 7). There are also chapters on ways of measuring wheat and flour quality, and improving flour for bread making. Part two reviews dough formation and its impact on the structure and properties of bread. It includes chapters on the molecular structure of dough, foam formation and bread aeration together with discussion of the role of key ingredients such as water. A final group of chapters then discusses other aspects of quality such as improving taste and nutritional properties, as well as preventing moulds and mycotoxin contamination.
