Abstract

Clinical Evidence summarises the current state of knowledge and uncertainty about the prevention and treatment of clinical conditions, based on thorough searches and appraisal of the literature. It is neither a textbook of medicine nor a set of guidelines. It describes the best available evidence from systematic reviews, RCTs and observational studies where appropriate, and if there is no good evidence it says so.Clinical Evidence is intended as a tool for clinicians, and thereby patients, to help them make evidence-based healthcare decisions. Our task is to provide information that is as accessible as possible without undue simplification. We aim to support the partnership between clinicians and patients that lies at the heart of good health care. We are working towards a future in which the information in Clinical Evidence can be personalised and synchronised with the electronic patient record, acknowledging that most interactions between patients and clinicians are complex and that 'evidence' is only one part of the equation.

Abstract

Evidence-based Medicine (EBM) is feared to become a kind of cook-book medicine that has nothing to do with the traditional skills and ethics. This volume shows the contribution EBM makes and might make to medical practice and health policy. It describes as many viewpoints as possible with a focus on the ethical issues that are at stake in this process. It shows how EBM has developed from an internal medical issue to an instrument for health policy. It is the outcome of the European Project "Ethical Issues of Evidence Based Practice in Medicine and Health Care" and gives insight into the ethical background of the debate on the role of EBM in various areas of medicine, including clinical practice, medical education, medical research, health policy and medical sociology.About the AuthorR. ter Meulen, PhDFull professor & Director of the Institute for Bioethics Department of Health Care Studies, Section Health Ethics and PhilosophyInstitute for Bioethicshttp://www.zw.unimaas.nl/gew/staffresdescr/ruudtermeulen.htmProf. Ruud ter Meulen (1952) is psychologist and ethicist. He is Director of the Institute for Bioethics and Professor for Philosophy and Ethics at the University of Maastricht and in Maastricht. The main research areas of Prof. Ter Meulen are the promoting of autonomy in the care for dependent elderly, resource allocation (particularly in regard to elderly care), medical ethics committees, ethical pluralism and the problem of consensus. Prof. Ter Meulen has directed several international projects. He was co-director of the International Project 'Care for the Elderly: Goals and Priorities' (1991-1994), a joint project with the Hastings Centre and sponsored by the European Union. He was coordinator of several European projects: the BIOMED project (1993-1995) 'Ethical Issues in the Research with Cognitively Impaired Elderly Subjects', the workpackage Ethical Issues and Quality of Life of the European Alzheimer Clearing House (EACH) project (1996-1997) and the project Ethical Issues of Elderly Care, which is part of the BIOMED project European Biomedical Ethics Practitioner Education (1996-1999) co-ordinated by Imperial College (UK). He was co-ordinator of the BIOMED 2 project 'Solidarity and care in the European Union' (1998-2000). He is coordinating two projects within the Fifth Framework. One of them is the project ‘Ethical Issues of genetics’ which is part of the TEMPE project (2000-2002), co-ordinated by Imperial College. The other one is the two-year project ‘Ethical Issues of Evidence Based Practice in Medicine and Health Care’ (EVIBASE) which finished in March 2003. Prof. Ter Meulen was principal coordinator of the EVIBASE project. He is partner and member of the core-group of the Project Bio-Technology-Ethics, coordinated by the University of Genoa (Prof. Celada), which develops a training course in ethics for PhD students in the field of biotechnology. Prof. Ter Meulen is the coordinator of NOMET, the national training course for members of ethics committees on the ethical and legal issues of medical research with human beings. This project is sponsored by the Ministry of Health and promoted by the Central Committee for Medical Research with Human Persons (CCMO).

Abstract

The Gold Standard is an engaging, well thought-out examination of a timely topic-evidence-based medicine. Timmermans and Berg raise taken-for-granted concepts such as power, objectivity, standardization, EBM and discuss them in innovative and complicated interpretations. The arguments presented here provide clarity on an important subject, highlighting the always interesting impact of unintended consequences." —Alexandra Dundas Todd, Department of Sociology, Suffolk University, and author of Double Vision, An East-West Collaboration for Coping with CancerFew things make people react more strongly to the changes going on in health care than the word standardization. Critics shudder at the mindless sameness of standards, while supporters dream of a world in which standardized "best practices" open up a world of efficient health care delivery. The Gold Standard takes up this debate to investigate the real meaning of standardization and how it affects patients, doctors, and the institution of medicine. Showing that standards are not about less or more skills, or more or less uniformity, but rather about a redefinition of autonomy, patients, and relationships, Timmermans and Berg show instead that they are about creating new worlds of medical treatment. Cutting through the hype and fears, the authors show where the true powers of standardization lie. The Gold Standard will become a classic for students of medicine and health care policy, and will be a welcome book for anyone concerned with the future of our system of care."The Gold Standard is an intelligent and well-written analysis of crucial developments in biomedical practice over the past century. Timmermans and Berg provide us with a comprehensive study of the growing movement to standardize biomedicine, calling attention to the risks and benefits for doctors and patients alike. This unique and important book should attract interest among medical sociologists, science studies scholars, bioethicists, health policy analysts, and all those who care about the practice of medicine today." —Steven Epstein, Department of Sociology, University of California, San Diego, and author of Impure Science: AIDS, Activism, and the Politics of Knowledge "This book sets the standard for the study of standardization in medicine. Focusing on evidence based medicine's meteoric rise and influence, Timmermans and Berg go beyond the professional rhetoric to examine its impact on the practice of medicine. Well-written and replete with sociological case studies, this book is must reading for social scientists, physicians and policy-makers." —Peter Conrad, Harry Coplan Professor of Social Sciences, Brandeis University "It is rare to find a book that can serve both as an excellent introduction to its subject for people who have not previously given it much thought or study and also as an advanced reading for people who know the subject area well. Happily, The Gold Standard is such a book." —The Journal of Legal Medicine "This eminently readable book is recommended to healthcare industry personnel and to policy makers who would be interested in a thoughtful study of the political, ethical, and functional aspects of the standardization of clinical behavior." —Quality Management in Health Care "[T]his is a surprising work...its scope places it in the realm of monumental works like Paul Starr's Pulitzer-Prize winning [book]. This book is highly recommended...especially to those grappling with the challenge of creating consistently high-quality health care at a cost our nation can afford." —Inquiry "...a highly readable and comprehensive book.... I can recommend this book to everyone who is interested in the subject." —Medicine Health Care and Philosophy: A European Journal (pdf) "This book is helpful to physicians and healthcare executives who need to have a better understanding of the evolution and application of evidence-based medicine." —The Journal of the National Medical Association.

Abstract

Abstract

"'Evaluating the impact of implementing Evidence-Based Practice' is the third of three books on implementing evidence based practice in the Evidence-based nursing series published jointly by Blackwell Publishing and Sigma Theta Tau International. 'Evaluation and outcome measures for evidence-based practice' addresses the means to rigorously identify, evaluate and assess the outcomes of implementation of evidence into practice. 'Evaluating the impact of implementing Evidence-Based Practice' provides practical and effective guidance on how to assess whether implementation was effective, the full range of issues to be considered when measuring the impact of implementation and the range of approaches which could be used. It stresses the importance of ensuring the long-term sustainability of improvements in care and of measuring the use of interventions and innovations, in order to effectively evaluate the impact on outcomes and. It examines the impact of implementation on economic outcomes and the outcomes of importance to different groups including service users, clinicians, managers and policy makers. The approaches outlined in each chapter are transferable across a range of international, professional and healthcare settings"--Provided by publisher.
