This report describes a comparative analysis of three proposals to allow Americans to keep their existing health plans under the Affordable Care Act (ACA). The proposals are evaluated based on their potential impact on the ACA-compliant market and the cost and coverage of health insurance. The possibility of each proposal causing a "death spiral" in the ACA-compliant market is also addressed.

The book focusses on three main topics: health inequity, self-responsibilisation and organisational reforms. Increasing inequity is one of the main outcomes of neoliberal policy in Europe and here the authors examine the impact of neoliberal policies on health inequality, providing a European comparative data analysis of healthy life expectancy and mental health issues in Spain. The book looks at self-responsibilisation, as part of neoliberal citizenship, through topics such as crowdsourcing medicine and citizen science. Finally, it analyses organizational reform in Europe using three case studies: Italian national health care reforms, mental health policy in Italy and maternal care in Russia. The book includes contributions from the Czech Republic, Italy, Russia and Spain and fosters the development of sociological debate in such countries within a European framework. It presents quantitative data analysis as well as ethnographic research and outlines a complex scenario affecting the everyday life of European citizens, their health and illness."

"In the 1960s and 1970s, Congress enacted a vast body of legislation to protect the environment and individual health and safety. Collectively, this legislation is known as "risk regulation" because it addresses the risk of harm that technology creates for individuals and the environment. In the last two decades, this legislation has come under increasing attack by critics who employ utilitarian philosophy and cost-benefit analysis. The defenders of this body of risk regulation, by contrast, have lacked a similar unifying theory." "In this book, the authors propose that the American tradition of philosophical pragmatism fills this vacuum. They argue that pragmatism offers a better method for conceiving of and implementing risk regulation than the economic paradigm favored by its critics. While pragmatism offers a methodology in support of risk regulation as it was originally conceived, it also offers a perspective from which this legislation can be held up to critical appraisal. The authors employ pragmatism to support risk regulation, but pragmatism also leads them to agree with some of the criticisms against it and even to level new criticisms of their own." "In the end, the authors reject the picture, painted by risk regulations' critics, of widely excessive and irrational regulation, but they do not entirely exonerate risk regulation either. The crisis of the Santa Barbara offshore oil spill in 1969 led to the passage of radically new environmental laws that made the United States an international leader in environmental protection at the time.
