Abstract

In orthodontic treatment, for many decades, panoramic radiographs and lateral cephalograms are considered the standard two-dimensional radiographic techniques required for treatment planning and follow up. Nevertheless, both imaging techniques present with several limitations such as geometric distortion and superimposition of anatomical structures. During recent years, there has been an upward trend in utilizing 3D images, especially from CBCT, as an aid in orthodontic diagnosis and treatment planning but the scientific evidence is still lacking in many aspects. Therefore, the primary goal of this doctoral thesis was to investigate the use of 3D images in orthodontics compared to conventional 2D modalities including panoramic radiography and lateral cephalography. Subsequently, an attempt was made to develop 3D reference systems to increase the reproducibility of several crucial cephalometric landmarks in 3 dimensions. Finally, the Frankfort horizontal plane was revisited, focusing more on its 3D version.This thesis begins with Chapter 1, explaining the general principles of orthodontic treatment planning and imaging modalities traditionally used to achieve the information needed to perform an orthodontic treatment. At the end of the chapter, the overall aims and hypotheses of this doctoral project were presented in detail. In Part I: Chapter 2, a systematic review on 3D cephalometry was presented. This systematic review focused on the scientific evidence for the diagnostic efficacy of 3D cephalometry, especially for landmark identification and measurement accuracy. It was clearly observed that this topic is fairly new and the scientific evidence of the diagnostic efficacy of 3D cephalometry is still limited and more concrete studies need to be performed. Methods of conducting research in this area are very crucial as radiation exposure to young patients is one of the main factors for ethical concern.In Part II, it was aimed to investigate and compare the use of panoramic radiography and the 3D data. In the first chapter of part II, Chapter 3, an attempt was made to compare in vitro subjective image quality and diagnostic validity of reformatted panoramic views from CBCT with digital panoramic radiographs, regarding orthodontic treatment planning. Results revealed that although digital panoramic radiograph still showed better image quality, some reformatted panoramic view from particular CBCT devices could achieve comparable image quality and visualization of anatomical structures. Next in this panoramic imaging part, the agreement between CBCT and panoramic radiographs for initial orthodontic evaluation was assessed. Chapter 4 showed that the agreement between CBCT and panoramic radiograph was good and CBCT could offer the same amount of information necessary for initial orthodontic evaluation.Subsequently, cephalometric imaging modalities were investigated in Part III of this doctoral thesis, beginning with Chapter 5. In thischapter, the linear measurement accuracy of three imaging modalities: two lateral cephalograms and one 3D model from CBCT data, was evaluated. The results showed better observer agreement for 3D measurements. The accuracy of the measurements based on CBCT and 1.5-meter SMD cephalogram was better than a 3-meter SMD cephalogram. These findings have confirmed that the linear measurements accuracy and reliability of 3D measurements based on CBCT data was good when compared to 2D techniques. Chapters 6, 7 and 8 concentrated on the reproducibility of cephalometric landmarks in 3 dimensions and attempted to develop a more robust system for 3D cephalometry. In Chapter 6, a new reference system was designed in Maxilim® software to improve the reproducibility of the sella turcica landmark in 3D. The results showed that the new reference system offered high precision and reproducibility for sella turcica identification in 3 dimensions.In Chapter 7, this time, a new reference system was developed in order to systematically improve the reproducibility of mandibular cephalometric landmarks (Pog, Gn, Me and point B) in 3D. It offered moderate to good overall precision and reproducibility for mandibular cephalometric midline landmark identification.Chapter 8 was the last study on 3D cephalometry in this doctoral thesis. The aim was to evaluate the Frankfort horizontal plane (FH), which is widely used in 3D cephalometric analysis. In this chapter, the precision and reproducibility of landmarks that form the Frankfort horizontal plane (Po, Or) and newly chosen landmarks (IAF, ZyMS) was investigated. The angular differences of optional planes compared to the Frankfort plane in 3D were measured. It was demonstrated that the precision and reproducibility of Po and Or was moderate. IAM and ZyMS showed good precision and reproducibility. From the newly proposed planes, the ones closest to the original FH are the plane formed by connecting Or-R, Or-L and mid-IAF (Plane 3) and the plane formed by connecting Po-R, Po-Land mid-ZyMS (Plane 4). This study demonstrated the possibility of using new planes when traditional FHs were not feasible.Lastly, in Chapter 9, the general discussion and conclusions were thoroughly discussed and presented. The findings of the present doctoral thesis elaborated the use of 2D and 3D images for orthodontic treatment and showed the possibility and new development to improve the use of 3D cephalometry. Although the scientific evidence on clinical use of 3D cephalometry is still limited, this project helps to provide a solid base for future studies. New studies should focus on the implementation of 3D cephalometry in clinical practice and evaluate how this new technology may improve the treatment outcome of orthodontic patients. In the near future, one ultra-low dose CBCT scan may be able to yield all necessary information and replace a cascade of 2D radiographic images while still keeping the ALARA principle. Whether those strategies are equally valid, remains to be proven.

Abstract

The effect of headgear on upper third molars: a retrospective longitudinal study. Objectives: To investigate the effects of orthodontic non-extraction treatment with or without headgear on the position of and the space available for upper third molars in growing children with class II malocclusions. Materials and methods: The sample consisted of pre– and posttreatment panoramic radiographs and lateral cephalograms of 294 class II orthodontic patients. 160 were treated with headgear, 134 were treated without headgear. The space available for the upper third molar was measured on the lateral cephalogram as the distance from pterygoid vertical (PTV) to the distal surface of the upper first molar crown (PTV-M1). Angulation, vertical position and tooth development stage of the upper third molars were evaluated on panoramic radiographs. All measurements were evaluated statistically. Results: In both groups PTV-M1 increases, but the increase in PTV-M1 was significantly higher for patients treated without headgear. A linear model for repeated measures revealed that this difference is still significant after correction for age, gender and molar occlusion. Further, there is no evidence that the change in angulation, vertical position and development stage of the upper third molars during orthodontic treatment is influenced by headgear therapy. Conclusion: This study indicates that the use of headgear in growing patients significantly affects the space available for upper third molars. However, orthodontic treatment with headgear does not influence the angulation, vertical position and development stage of upper third molars. It is therefore important to always take into account third molars during treatment planning. The effect of first and second premolar extractions on third molars: a retrospective longitudinal study. Objectives: to analyze the effect of first and second premolar extractions on eruption space for upper and lower third molars and on third molar position and angulation during orthodontic treatment. Methods: the sample consisted of 296 patients of which 218 patients were orthodontically treated without extraction and 78 patients with extraction of first or second premolars. The eruption space for third molars was measured on pre– and posttreatment lateral cephalograms, whereas the angulation, vertical position, the relation with the mandibular canal and the mineralization status of third molars were evaluated using pre– and posttreatment panoramic radiographs. All data were statistically analyzed. Results: the increase in eruption space and the change in vertical position of upper and lower third molars significantly differed between patients treated with and without premolar extractions, whereas the change in angulation, relationship with the mandibular canal and mineralization status of the third molars did not significantly differ between patients treated with and without premolar extractions. Conclusions: The retromolar space and the position of third molars significantly change during orthodontic treatment in growing patients. Premolar extractions have a positive influence on the eruption space and vertical position of third molars, whereas they do not influence the angular changes of third molars. Clinical significance: This study stresses the importance of considering the possible effects of orthodontic treatment on third molars during treatment planning.

Abstract

ABSTRACT Introduction: Adequate comprehension of patients' facial preferences and orthodontic treatment needs is fundamental to the success of orthodontic treatment. The manifestation of proclined maxillary and mandibular incisors is a prevalent facial feature that may present as a singular or combined issue with other dental and skeletal problems. Hypothesis and aims: To gain knowledge of the inclination of incisors and their impact on orthodontic treatment decision-making. Materials and methods: Consequently, this review examined studies available in English, French, and Dutch in five electronic databases (i.e., Pubmed, Embase, Web of Science, Scopus, and Central) that discussed the cephalometric analysis and facial features in patients with proclined maxillary and mandibular incisors. Results: Several factors are considered when selecting the appropriate orthodontic treatment, as discussed in this literature review. The results revealed the influence of these proclinations on the position of dentoalveolar structures, patient profile, and occlusion, as well as the concept of dentoalveolar compensation and the ideal treatment approach based on the class of malocclusion. Conclusions: The review concludes that a comprehensive plan, integrating both the orthodontic and surgical components, should be established to account for facial features and cephalometric measurements, as this approach has yet to be adequately formulated.

Abstract

Introduction: This study aims to find a standardized method to describe the changes in the arch perimeter, so it could be added to the protocol of the expansion study. The previous study aimed to investigate the effect of interceptive orthodontics on the dental arch characteristics after slow maxillary expansion. No method to determine the arch perimeter has been added yet to the existing protocol. Material and method: A literature review was conducted for a standardized method to determine the arch perimeter before and after expansion. Furthermore, the influence of the inclination of the upper incisors on the arch perimeter was investigated through a literature review to determine whether this should be taken into account for a standardized method for measuring the arch perimeter. These findings were gained by measuring the difference of the arch perimeter and the inclination of the upper incisors before and after treatment. Pearson’s correlation was used to check whether there is a correlation present with the help of SPSS statistics. The test group consists of 9 individuals who had a cervical headgear as treatment to distalize the molars, so the incisors have the chance to change the inclination. Results: Fifteen different methods were found to determine the arch perimeter. These were just an estimation of the arch perimeter and did not take into account the different criteria which could have an influence on the arch perimeter. Only 7 relevant articles about the relationship between the inclination and the arch perimeter were found, they were all unreliable. The control measurements showed no correlation between the change of the arch perimeter and the change of the inclination. This result was not significant due to the small sample size. Conclusion: It can be concluded that no standardized method exists. So, it is not possible to add one to the protocol of the expansion study. The research for the effect of the inclination of the upper incisors on the arch perimeter shows no reliable results.

Abstract

1. The 22q11.2 deletion syndrome (22q11.2DS) is one of the most frequent microdeletion syndromes and presents with a highly variable phenotype. In most affected individuals, specific but often subtle facial features can be observed. Here we aim to thoroughly investigate the craniofacial and dental features of 20 children with a confirmed diagnosis of 22q11.2DS by analyzing 3D facial surface scans, 2D clinical photographs, panoramic and cephalometric radiographs and dental casts. 3D facial scans were compared to scans of a healthy control group and analyzed using a spatially-dense geometric morphometric approach. Cephalometric radiographs were digitally traced and measurements were compared to existing standards. Occlusal and dental features were studied on dental casts and panoramic radiographs. Interestingly, a general trend of facial hypoplasia in the lower part of the face could be evidenced with the 3D facial analysis in children with 22q11.2DS compared to the controls. Cephalometric analysis confirmed a dorsal position of the mandible to the maxilla in 2D and showed an enlarged cranial base angle. Measurements for molar occlusion, overjet and overbite did not differ significantly from standards. Four of the 20 patients presented with tooth agenesis. Despite individual variability, we observed a retruded lower part of the face, a tubular nose, a small mouth and an increased lower facial height as common features and we also found a significantly higher prevalence of tooth agenesis in our cohort of 20 children with 22q11.2DS. Furthermore, 3D facial surface scanning proved to be an important non-invasive, diagnostic tool to investigate external features and the underlying skeletal pattern. 2. Aim: To systematically review the existing literature about 3D imaging in patients with craniofacial syndromes, evaluate its quality and propose some recommendations for future research. Method: PubMed, Embase and Cochrane databases were electronically searched. Inclusion criteria were patients with craniofacial syndromes and 3D imaging of facial soft and/or hard tissues. Exclusion criteria contained of non-genetical conditions, injury or trauma, facial soft and hard tissues not included in the image analysis, case reports, reviews, opinion articles. No restrictions were made for patients’ ethnicity or age, publication language or publication date. Study quality was evaluated using the Methodological Index for Non-Randomized Studies (MINORS). Results: The search yielded 2124 citations of which 113 were assessed in detail and 57 were eventually included in this review. Studies showed a large heterogeneity in study design, sample size and patient age. An increase was observed in the amount of studies with time and the imaging method most often used was CT. The most studied craniofacial syndromes were Treacher Collins, Crouzon and Apert syndrome. The articles could be divided into three main groups: diagnostic studies, evaluation of surgical outcomes and evaluation of imaging techniques. MINORS scores ranged from 3 to 11 over 16 for non-comparative studies and from 10 to 18 over 24 for comparative studies. Conclusion: The general quality of the included articles was moderate to low and prospective, controlled trials with sufficiently large study groups are lacking. To improve the quality of future studies in this domain and given the low incidence of craniofacial syndromes, more prospective multi-center controlled trials should be set up.
